Dear Mr. Chao:
In accordance with your letter (Doc ID 19843) dated September 30, 2013, this letter is being provided as a
written request for permit modification of the subject facility's Permit to Operate. This request is being
made on behalf of Republic Services of North Carolina, LLC. Enclosed you will find a copy of the
Amendment to Disposal Service Agreement #6 for the subject facility. Please note that this Agreement
extends the term for a period of approximately 7 years, or until July 31, 2023.
At this time, we are requesting that a Life of Site Permit be issued for this facility, concurrent with the
Amended Agreement. We understand this facility has converted to an annual fee system and that no
permitting fee is required for this permit modification. Thank you for your assistance with this matter.
