The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Application for permit received on May 23, 2016. Site evaluation was delayed due to a compliance
issue discovered against S & S Septic Tank Pumping, Permit # NCS-00282, on August 12, 2016.
2) Site evaluation conducted today for permitting of area identified in application for septage land
application. Evaluated two fields for septage applications. Both fields had been established in pine
trees. The soils and setbacks were checked for compliance according to NC Septage Management
Rules. Aerial map to be included with permit to show the area of Fields1 and 2 that meets the Septage
Rules.
3) Septage Land Application Site and No Trespassing signs present. Will need to add another sign along
the access road by the storage tanks.
4) Trees were flagged marking the outside edges of the two fields. Once permitted, septage may be
applied within the two areas marked as Field 1 and 2. The trees flagged need to be marked more visibly
and permanently with something like paint. Enough trees need to be marked so that the boundaries of
the two fields are clearly identified. Please asked for assistance (John Farnell or myself) if unsure of
which trees to mark.
5) Please keep the understory vegetation under control.
6) The monthly application rate of ‘Low” in the nutrient management plan was adjusted to approximately
1,900 gallons/acres to allow for equal monthly application throughout the year.
7) Checked location for the placement of two – 7,000 gallons steel tanks for septage storage. The tanks
will need to be permitted as a Septage Detention and Treatment Facility. Application for permit received
on May 23, 2016.
