Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1-Inspected vehicle in regards to issuing first firm permit. Tag#YA138377, VIN 1HTSDTVN1LH264372, 2600 gallons Tank appears structurally sound and securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of the vehicle. 2- I informed Corinne Law/Raleigh central office that the vehicle is in compliance and that Mr. Byron wanted to be contacted when the firm permit was issued. No septage jobs shall be performed until the permit is issued. 3-Mr. Hunt stated that he has the example truck log to begin using once the firm permit is signed. Discussed septic tank and portable toilet pump jobs. Mr. Hunt stated that he may obtain a discharge authorization from PWC in Fayetteville. I gave Mr. Hunt a copy of the 2017 class schedule & discussed attending a training class before the end of the year so that the firm permit may be renewed this winter. 4- Discussed required setbacks for a septage land application site.
