1. All photographs were taken by Troy Harrison on November 16, 2016.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2. The Septage Land Application Site consists of four fields totaling approximately 30.17acres. Field 5A contains
8.01 acres, Field 5B contains 4.88 acres, Field 6A contains 9.74 acres and Field 6B contains 7.54 acres.
Field 6b 3. The site is permitted for land application of domestic septage and grease septage.
4. The site is located at the end of Penn Road and has accessible all-weather roads to the site and septage land application fields covered under the permit.
5. Appropriate signage was displayed at the site. 6. Disposal area boundaries were delineated with permanent markers. During a previous inspection, on the west
side of field 6a, the pond was full and there was not a 100 foot buffer to the pond. The boundary markers were observe to have been adjusted so that when the pond is full the 100 foot buffer is maintained.
7. All septage discharges appear to have been made within the disposal area boundary.
8. There is no evidence of standing liquid or pooled waste. The waste appeared to be evenly distributed throughout
the field.
9. No objectionable odors were observed.
10. Field 6b had recently been tilled. As a reminder, Fields cannot be used for land application unless an adequate fescue crop is established and maintained. In addition, the approved Nutrient Management
Plan contained in your permit has fescue as the cover crop for all four fields. Planting of cover crops other than fescue is not approved in the current Nutrient Management Plan. Please contact Connie Wylie at connie.wylie@ncdenr.gov or 910-433-3352 to update the nutrient management plan. You can also contact the Rockingham County Cooperative Extension at 336-342-8230 for advice on field maintenance and weed control.
