The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Septage Land Application Site consists of Field 1 (8 acres) and Field 2 (8 acres).

2. The site is permitted for land application of domestic septage and grease septage.

3. The site is located at 707 Poplar Springs Church Road and has accessible all-weather roads to the septage land
application field covered under the permit.

4. Appropriate signage was displayed at the site.

5. Disposal area boundaries were delineated with permanent markers.

6. All septage discharges appear to have been made within the disposal area boundary

7. No Standing liquid or pooled waste was observed. The waste appeared to be evenly distributed throughout the
field.

8. No objectionable odors were observed.

9. The approved Nutrient Management Plan contained in your permit has fescue as the cover crop for all four fields.
Planting of cover crops other than fescue is not approved in the current Nutrient Management Plan. The next
permit renewal is to include other cover crops. If you have not already done so, please contact Connie Wylie at
[e-mail] or 910-433-3352 to update the nutrient management plan. You can also contact the
Cleveland County Cooperative Extension at 704-482-4365 for advice on field maintenance and weed control.
