For the discussions in the upcoming pre-operational meeting on this Friday, 08/11/2017, I send you the draft
comments (DIN 28384) on the CQA Report for the construction of Cell 14B (DIN 28372). The comments are
stated below:

1. (Section 4.3 & Section G-4) Please correct the likely typographic errors in Section 4.3:
CL-43R1 should be CL-43R2 and CL-72R1 (a duplicate) should be CL-71R1.

2. (Section 5) Please provide additional info or clarification requested below:
i. (Section 5.1) Please provide a copy of manufacturer’s 20-year material warranty of the 60-mil
HDPE [Technical Specification Section 02750, Paragraph 1.04].
ii. (Section 5.1) Are the constructed geomembranes smooth or textured 60-mil HDPE liners or both
(textured one placed on the sloped areas and smooth one placed on the flat areas)? Please
clarify.
iii. (Sections 5.2 & 5.3) Have the rolls of geotextile been tested and inspected for broken needles by
the manufacturer or the CQA Engineer? Please clarify or provide a manufacturer’s
certification(s).

3. (Section 7) Please provide additional info or clarification requested below:
i. Three options of the protective cover were approved in Technical Specifications for Phase 4
construction (May 2014, DIN 23531), please describe which option is selected and constructed at
Cell 14B.
ii. The stated ASTM testing methods for calcium carbonate content (ASTM D 4373) and
permeability (ASTM D5084) in Section 7.1 are different from those (ASTM D3042 & ASTM
D2434, respectively) used in the laboratory testing presented in Appendix S. Please clarify.
iii. This Section shall report the completed tasks/activities associated with leachate piping
installation according to Technical Specifications – Sections 02745 & 02748 and Rule 15A
NCAC 13B .1624(b)(11)(C). The specified physical properties of the piping material from
manufacturers and/or laboratories should be provided in the CQA Report.

4. (Section 8 and As-built Drawings)

i. Are the grid elevations presented on the Figures 1, 2, & 4 the measurements being perpendicular
to the surfaces – sloped and flat areas as required in the Technical Specification 01050? Please
clarify.

5. (Appendix C) According to Rule 15A NCAC 13B .1621(b)(4), the meeting minutes and sign-in sheet
associated with Cell 14B construction should be appended to this CQA report. The meeting minutes
includes pre-construction, progress, or troubleshooting meetings

6. (Appendix G, Section G-4)
i. The in-situ moisture content (31.6%) of sample CL-1 exceeds the specification [at or above 3% wet
of the optimum moisture content (28.3%)]. The reported difference of moisture of +2.3% is a
typographic. Please make a correction.
ii. The remarks for sample CL-70R1, CL-71R1, and CL-72R1 are likely incorrect. Please clarify.

7. (Appendix G, Section G-5) Please add the sample location CL-13R1 to the drawing.

8. The CQA Report should include the tasks/activities and testing result data stated in the approved work
plan (DIN 27540) and the updated Technical Specification – Section 02220 dated September 2016
regarding construction of an underdrain underneath the Cell 14B. Please provide the requested testing
result data and as-built drawing.

60 mil Microspike HDPE Geomembrane. This is noted in Section 1.1

Response: Enclosed are copies of the needle free certifications from the

Response: The protective cover option used in the construction of Cell 14B is
provided in Technical Specification 02254 – Protective Cover – On
Liner – Gravel / Sand of the referenced document. In the cover letter
of the above referenced submittal, the system used is more adequately
described as Option No. 2.
The protective cover material used is also described in Section 1.1 of
the CQA report.
ii. The stated ASTM testing methods for calcium carbonate content
(ASTM D 4373) and permeability (ASTM D5084) in Section 7.1

Response: The noted discrepancy in ASTM standards was a typographical error

Response: The specification sections referenced are not included in the Cell 14B
Construction documents. Technical Specification 02747 covers the
leachate collection system for this project, but the other 2
specifications are not used. The rule referenced discusses
requirements for CQA testing associated with the leachate pipe, and
does not appear to be applicable to this project.
As discussed at the final inspection meeting, we have enclosed a copy
of the leachate piping submittal provided by the Contractor. This
submittal confirms the materials that were used for this project.
4. Comment: (Section 8 and As-built Drawings)
i. Are the grid elevations presented on the Figures 1, 2, & 4 the
measurements being perpendicular to the surfaces – sloped and
flat areas as required in the Technical Specification
01050? Please clarify.
Response: The measurements shown on the as-built Figures 1, 2, & 4 are the
actual measurements surveyed at the ground surface. Minimum
required thickness must be achieved in the perpendicular
direction. Therefore, comparing elevations at the certification points
(in the vertical direction) requires the thickness to be slightly more
than the minimum required, as noted in the comment below.
ii. If the answer of the previous comment is “no,” then please re-
examine/confirm the survey data on Figure 2 - Top of Clay; two
locations have the measured thicknesses of the in-place compacted
clay liner less than the minimum of 2-feet thickness (perpendicular
to the side slope).

Response: We are aware of these two locations, and the apparent thickness
deficit of .02 feet and .03 feet, respectively. In both cases, these 2
locations were identified by the project engineer, and confirmed in the
field by the CQA firm through thickness verifications augured by
hand and measured in the field. This information was recorded on
the Daily Reports, dated: 6/7/17 and 6/10/17 which are provided in the
CQA report.
ii. (Figure 4 -Top of Protective Cover) Several locations (at or
adjacent to the leachate collection pipes) don’t have the
measurements/elevations of the constructed protective cover
layer. Please provide survey data.
Response: The certification points that do not provide top of gravel elevations on
the referenced as-built fall along the temporary rainflaps and do not
have sufficient gravel in place until the rainflaps are removed and the
stone under the rainflaps is pushed into place. This rainflap removal
will be documented by the site operations team with copies placed in
the Facility Operating Record in accordance with the Facility’s
Permit.
iii. Please provide the acreage of the constructed liner area of Cell
14B on the as-built drawing [Technical Specification Section
01050, Paragraph 3.04].
Response: The acreage of the constructed liner has been added to the as-built
drawing.
iv. If the rain flap (on Figure 4) is used as the storm-water separation
device in Cell 14B operation, please show the extent of the rain
flap to be covered on the drawing.

Response: There were 2 temporary rain flaps installed as storm-water
separation devices inside Cell 14B, as shown on Figure 4. Both flaps
will be removed (cut out) by Republic personnel prior to waste filling
above these areas. This removal will be conducted in accordance with
the facility’s current permit.
5. Comment: (Appendix C) According to Rule 15A NCAC 13B .1621(b)(4), the meeting
minutes and sign-in sheet associated with Cell 14B construction should be
appended to this CQA report. The meeting minutes includes pre-
construction, progress, or troubleshooting meetings.
Response: The pre-construction meeting minutes, including sign-in sheet, have
been attached. Formal minutes for progress or troubleshooting
meetings were not prepared during the course of this project.
6.Comment: (Appendix G, Section G-4)

final project ship date when subsequently properly installed, covered and used for a LANDFILL(BURIED).
AGRU warrants that the liner will be free from material defects and is manufactured in all material
respects to AGRU’s product specifications as indicated in the applicable AGRU technical records,
catalogs, guidelines, and test certificates in effect at the time when the liner is sold. Note: AGRU’s
products may vary in details of design and construction from descriptions in any literature or from any
sample, display, or other model inspected by Customer.
AGRU disclaims all other representations and warranties of any kind, express or implied, in fact or in
law, including, without limitation, the implied warranty of merchantability and the implied warranty of
fitness for a particular purpose. Every claim under this limited warranty shall be deemed waived unless
in writing and received by SELLER within 10 days of delivery if visibly damaged or defective, and,
otherwise, within 30 days after the defect to which each claim relates is discovered, or should have been
discovered, but in no event longer than TWENTY(20) YEARS after product shipment.
AGRU’s liability under this limited warranty is not applicable when damage is caused by: Natural
phenomena such as, but not limited to: thunderstorms, floods, earthquakes, or other acts of God; and
acts of war, machinery, foreign objects or animals; and Chemicals which are not suitable for HDPE liner
materials.
AGRU’s Limited Material Warranty will be void if any of the material is shipped prior to completion and
approval of all testing required by the project specifications.
Further, the AGRU limited warranty is voided by, and AGRU in no event shall be liable for damages due
to improper site preparation, as well as the misapplication, incorrect installation (including incorrect
welding of seams in the installation), and/or damages resulting from any kind of improper handling.
Further, AGRU’s warranty will be void in the event that the Customer performs repairs or makes
alterations without the express approval of AGRU in writing. Customer shall not repair, replace,
remove, alter, or disturb any liner prior to AGRU’S inspection except that the Customer may take
emergency action necessary to prevent damage to persons, property or the environment. Failure to
comply with this paragraph shall void the warranty.

This warranty is only valid on condition that the generally approved technical standards, and in
particular the guidelines for the installation of the liner, are followed in all respects.
AGRU shall be given an opportunity to ascertain the cause of damages. AGRU reserves the right to
decide how damages will be settled. Customer agrees that it shall provide AGRU with clean, dry and
unobstructed access to the liner in order for AGRU to perform the inspections and repairs which may be
required pursuant to the warranty. AGRU shall not be obligated to perform any inspection or be
obligated to perform any repair or replacement under this warranty until the area is made available free
from all obstructions, water, dirt, sludge, residuals and liquids of any kind. AGRU shall not be liable for
any costs relating to providing such access to the liner. If after inspection it is determined that there is
no claim under this warranty, Customer shall reimburse AGRU for its costs associated with the site
inspection.
Customer’s exclusive remedy and the limit of AGRU’s liability for breach of this limited warranty,
whether based on negligence, breach of warranty, strict liability, or any other theory, at law or in equity,
shall be, at AGRU’s option: repair, replacement with a like quantity of non-defective product, or refund
of the purchase price, plus reasonable handling and transportation charges incurred for approved
refunds.
AGRU will not be held responsible for any delays in performing approved material repairs or
replacements that are caused by machinery or mechanical failure beyond AGRU’s control.
AGRU’s maximum liability under this warranty will not exceed the purchase price of liner and will only
be in force when payment has been made in full, and further claims, regardless of the legal suppositions,
are not applicable.
All AGRU products (including liners) are sold under AGRU’s General Terms and Conditions, including this
Limited Material Warranty, and bar on recovery of consequential and other damages.
AGRU AMERICA SHALL NOT BE LIABLE FOR ANY CONSEQUENTIAL OR SPECIAL DAMAGES BASED ON
NEGLIGENCE, BREACH OF WARRANTY, STRICT LIABILITY, OR ANY OTHER THEORY, FOR FAILURE TO
PERFORM ITS OBLIGATIONS UNDER THIS AGREEMENT. ADDITIONALLY, CONSEQUENTIAL AND SPECIAL
DAMAGES SHALL NOT BE RECOVERABLE EVEN IF THE REPAIR, REPLACEMENT, OR REFUND REMEDY FOR
AGRU AMERICA’S BREACH OF ITS LIMITED WARRANTY FAILS ITS ESSENTIAL PURPOSE OR FOR ANY
OTHER REASON.
No warranty made by any representative of AGRU, or any other person, regarding the AGRU material
shall be binding upon AGRU except the warranty set forth herein.

The meeting was held in the office of the Uwharrie Regional MSW Landfill. The meeting was conducted by Mr.
Matt Cheek, P.E.

I. INTRODUCTIONS:

A. Identify Parties and Responsibilities:

Liquidated Damages have been set at $1,500 per day per the contract documents.

6. CQA Surveying-“As-Built” Surveying Services: Wright & Fields Land Surveying of Troy,
NC, Tommy Fields, RLS. Wright & Fields will perform a preconstruction survey. The
preconstruction survey will be used as the official survey for determining pay quantities. Mr.
Fields will flag wetland buffers and clearing limits in Borrow Area No. 3. HHNT and
Wright and Fields will produce the 50’ x 50’ record survey grid. As-Built surveys will be
performed as follows:

1) A survey of current conditions is being prepared by Wright and Fields. Some
modification of the contract quantities for earthwork may result.

2) Top of subgrade survey.

3) Top of clay liner survey.

4) Installed HDPE geomembrane survey (Panels, destructive samples and edge of
liner only.)

5) Top of protective cover survey. Including all piping, edge of cell markers and
completed site grading. Tommy will survey leachate piping where it is exposed.

6) Final As-Built Survey.

NOTE: As-built surveys may be coordinated directly with the surveyor, but all parties should
be informed of scheduled visits. Mr. Fields requires a minimum 3 days’ notice prior
to performing confirmation surveys. The Owner may use the on-site GPS system to
survey certain items. It was noted that subgrade, clay, and protective cover surfaces
must be constructed within specified tolerances. There is no excess of clay on this
site and this resource must be managed carefully.

NOTE: The owner will pay for up to 2 separate trips for as-built certification of each surface
by the surveyor. Additional trips may be charged back to the contractor.

7. Construction Surveying: The construction surveyor will be the responsibility of Preston.
Preston intends to use their in house staff for construction surveying services. Preston may
not use Republic Services GPS system base station. Slope thicknesses will be measured
perpendicular to the slope, not vertically. Preston must use the same survey grid and control
as the certification surveyor. Preston must check into the on-site control system at the
beginning of the project. Any discrepancies should be brought to HHNT’s attention
immediately.

8. North Carolina Department of Environmental Quality (NCDEQ): The NCDEQ staff for this
project is listed as follows:

Permitting Engineer and Project NCDEQ Contact: Ming Chao, P.E.

Compliance Inspector: Teresa Bradford

Permit Hydrogeologist: Christine Ritter

Ming Chao, P.E. will be the NCDEQ project contact and will review all correspondence
including the CQA report. The CQA report is to be submitted in hard copy (2 copies) and
following approval of the hard copies, 1 CD in PDF format. It will be posted on the NCDEQ
website. The NCDEQ is to be notified prior to any required rock blasting. NCDEQ approval
of the blasting is required prior to blasting in the cell area. Preston should not contact the
NCDEQ staff for any reason. All coordination with the State should go through HHNT or
Owner’s Project Manager.

II. LINES OF COMMUNICATION:

A. All project communication must be routed through HHNT. HHNT will distribute information to the
various project parties. Information should be provided in writing via email or memos to provide
written documentation. Each party should establish a single point of contact for both field and office,
as applicable.

B. On-site activities regarding selection of material laydown areas, stockpiles, haul roads and
interaction with the existing landfill operations will be directed to Joe Reynolds, the Site Manager.

Route all paperwork through HHNT who will maintain the project file and distribute documents.
Mr. Matt Cheek, P.E. is the certifying engineer.

Contract-related issues will be decided by Mike Gurley

Technical issues will be directed to Matthew Cheek, P.E.

Joe Reynolds is to be contacted upon the arrival of NCDEQ personnel or Montgomery County
Personnel so that he may accompany them while on site.

C. Progress Meetings: Meetings are to be held on-site. Progress meetings are tentatively scheduled to
be every other Wednesday at 10:30 AM starting on 2/8/17 and will be set up by ACC and Preston
with the other project parties notified for potential attendance. The on-site CQA representative is to
have a daily “tailgate” meeting with the Preston project superintendent. Once a month project
meeting minutes are to be recorded by ACC. Matt Cheek, P.E. and/or Kevin Berry, P.E. will attend
meetings on a periodic or as need basis. A pre-FML deployment meeting is to be conducted by the
CQA Engineer.

D. Design Changes during Construction:

1. Change Orders: Requests for change orders are to be submitted to HHNT in writing. All change
orders require written approval by HHNT and Republic Services even if schedule or cost
changes do not result. The CQA technician is not authorized to direct the work of the contractor
or make changes to the design. Written change orders must be in-place for an item to be on a
pay request.

2. Verbal Changes: Verbal authorization to proceed with an item resulting in a change order may
be provided to the contractor by HHNT or Republic Services, but must be backed up with a
written change order prior to submission for payment. A written change order is required even if
payment modification is not part of the change order.

3. Payment Requests: Monthly pay requests are to be prepared and submitted, in color, to Mike
Gurley by the 25th of each month for processing via PDF email (send draft by 21st/22nd of each

month for initial review). All pay requests are to be free of math errors or they may be rejected.
Note that soil quantities will be estimated on a percentage of completion basis for payment
requests. Only one copy will be required for each pay request. Payment will be made for
materials stored on-site. Payment will be made based on actual quantities. Any payment issues
that arise should be directed to Mike Gurley. An updated construction schedule should
accompany all pay requests.

4. No Cost Change: Includes moving items and time extensions. Change orders must be fully
executed prior to pay requests.

5. Submittals: Should be provided to Matt Cheek, P.E. at HHNT and will be reviewed and returned
with any comments. Preston will be responsible for issuing submittals in accordance with the
contract documents. Electronic copies signed and sealed by the contractor will be accepted as
long as the scans are clear and of good quality. Warranty documents are to be submitted as
required for all products. Approval of a submittal does not relieve Preston of the requirement to
meet the project plans and specifications.

III. DISTRIBUTION OF DOCUMENTS:

IV. SITE REQUIREMENTS:

A. Safety Rules: Preston is required to follow the Republic Services safety standards at a minimum.
Job site safety is the sole responsibility of the contractor. Any suggestions, comments or notations
provided by representatives of Republic Services, HHNT or the CQA firm are for information only
and do not relieve Preston of their responsibility for safety. A safety plan submittal is required.
Preston has signed the Republic Services Site Safety Agreement. The project site requires workers to
wear a hard hat when overhead hazards are present, work shoes, a reflective vest and long pants. No
gun fire is allowed. The design Engineer (HHNT) may shut down the job if safety issues become a
concern. Preston maintains a no cellphone policy for employees operating equipment. Republic
maintains a site wide speed limit of 15mph. Republic’s Contractor’s Safety Declaration form must
be signed by all contractors and subcontractors.

B. Security: Preston will utilize the same lock as the landfill for the Contractor’s Entrance Gate. A key
will be provided and copies made by Preston. Preston should secure all equipment and take
precautions against vandalism. Republic’s night watchmen will monitor the entire site, but Republic
accepts no responsibility for damage to the contractor’s equipment.

C. Contractor Facilities: To be coordinated with Joe Reynolds.

D. Traffic Control and Parking: To be coordinated with Joe Reynolds. The public has access to the yard
waste area and use of the access road. Dust is to be controlled by water application. The Owner will
notify residents coming onto the site of the ongoing construction. The landfill operating hours are
6:00 AM to 5:00 PM.

E. Erosion Control: Erosion control is a sensitive issue and is to be handled in accordance with the
project documents. The site stormwater is directed to surface water streams that must be protected.
Preston is responsible for required stormwater run-off control and monitoring. Should Preston
determine that additional controls are needed to control sediment, those measures should be
implemented and HHNT should be notified. Brian Scott will provide E&S inspection forms that shall
be completed by Preston during construction.

F. Landfill Offices & Shop: A fax machine is available in the landfill office (when open) for use by
Preston and ACC. Sanitary facilities for Preston’s crew will be provided by Preston.

V. CONSTRUCTION:

A. Scope of Work: The scope of work includes the construction of Cell No. 14B as described in the
project documents. The liner system consists of a prepared subgrade, a base liner system consisting
of a 24-inch thick compacted clay liner (K < 1 x 10-7 cm/s), minimum 60 mil textured geomembrane,
12 osy geotextile fabric and 24-inches of protective cover. The following geosynthetic materials will
be provided by Republic. This is a unit priced; line item job and the Owner will pay for measured, in
place Quantities.

The 12 osy geotextile may be doubled up under the leachate stone column to provide the required 24 osy
geotextile cushion.

Preston is responsible for maintaining the protective cover stockpile. All of the protective cover material
shall be provided by Republic. The material laydown areas or staging areas should be coordinated with Joe
Reynolds. Note that 4-foot thick haul roads are required under all rubber-tired equipment when placing
protective cover soils in the cell area.

This project also includes work in wet detention basin 7B (material removal only), placement of rocky fill in
proposed berm and construction of turn around pad.

B. Construction Procedures:

1. The area required to obtain clay liner borrow should be identified early on in the project. A Land
Disturbance Permit has been obtained. Any clay identified within the cell areas should be
undercut and removed for liner use. Efforts should be made early in the project to identify and
quantify the clay material available on-site. HHNT should be notified immediately if shortages
are expected so that additional areas can be permitted for land disturbance.

2. Structural Fill: This project includes some deep structural fill areas. Boulders and large rocks
may be placed in these areas, but they must be placed to provide a stable fill. No boulders or
large rocks will be allowed in the top 3’ of any fill areas.

3. Rock: Rock is defined in the technical specifications and requires ripping attempts using a D-8
dozer with a single tooth ripper. If rock is encountered and the conditions approved by HHNT,
blasting may be required. Blasting will require submittal of a formal blast plan to HHNT for
notification and approval by NCDEQ. The actual quantity of rock to be removed in the Basin 7B
Area shall be determined early on in the project. Authorization by the Owner must be obtained

prior to exceeding the rock excavation quantity on the bid form. The Owner requests that prior to
a blasting, the Contractor’s Seismologist handle neighborhood notifications. A call notification
may also be sent out through the County Manager’s Office.

4. Clay Liner Test Pad: A clay liner test pad will be required and should be completed in
accordance with the specifications. It will consist of 4 lifts installed using the same construction
methods that will be used to construct the production clay liner for Cell No. 14B. The test pad
may be used to experiment with the clay liner compaction and moisture content. The final lift, as
a minimum, must meet the project density, moisture and permeability criteria. A finished
surface of the top of clay liner test pad suitable for FML installation and approved by the CQA
firm is required. No dragging will be allowed. The test pad soils may be used in construction of
the soil liner, but only after reprocessing and recompaction and not as a finished product.

5. Screening: Previous jobs have not required the clay liner material to be screened, but the
material must meet the specifications for particle size. The clay liner material may also require
hand picking to remove debris or unacceptable size rock.

6. Certification Surveys: Both grid points and contour lines will be checked and all must indicate
the required thickness has been achieved and design grades have been achieved. Thickness can
be checked by ACC with confirming hand-augers.

7. Equipment: Preliminary equipment list to be provided to the Owner by Preston.

Fuel and maintenance of equipment is to be done outside of Cell No. 14B and any future
construction areas and at least 100’ from ground water monitoring wells. Fuel storage tanks are
to be dual contained. No fuel storage allowed in future cell areas and is to be away from
groundwater or methane monitoring wells. (Preston anticipates using a tank with a capacity of
3,000 gallons, at a minimum). If a spill occurs, it is to be excavated, mixed with soil as required,
and disposed of in the landfill working face and the landfill is to be notified immediately so that
proper reports can be completed and the excavation and disposal performed in accordance with
the site requirements. No spill is too small. All machines and equipment should be inspected for
leaks regularly and repaired as soon as possible.

8. Division of Responsibilities Among Contractors: Preston is responsible for site coordination
among the various material suppliers or other contractors. The Owner will provide the HDPE
geomembrane, and 12 osy cushion geotextile fabric to cover the geomembrane. Preston is
responsible for the off-loading and storage/staging of all delivered materials. Preston is
responsible for maintenance of the stone stockpiles. Republic Services (Mike Gurley) is
responsible for requesting the geosynthetics installer to be mobilized to the site. The landfill is to
be notified of material delivery schedules. Preston will be responsible for maintaining the clay
liner surface during FML deployment by the Geosynthetics contractor. The Owner requires that
all material deliveries be scheduled during times when Preston is on-site.

FML deployment will be conducted by Environmental Specialties International, Inc. The
contract documents specify that 20 days out of the contract time are designated for liner
deployment.

9. Construction Schedule: Preston to submit a construction schedule and provide periodic updates.

10. Construction Dewatering: Preston is responsible for all construction surface and groundwater
control including run-off from the active landfill cells. Preston should discuss their plan with
HHNT and the site.

11. Work schedule / hours open: Preston plans to work Monday through Friday from 6 AM to dark
and Saturday from 6 AM to 4 PM. Work after 4 PM on Saturdays will not be allowed, however
maintenance activities may be performed. No site work except liner installation is allowed on
Sunday. Mr. Reynolds requested that no one be on-site by themselves for safety reasons. Mr.
Reynolds requested that consideration regarding construction noise be made for adjacent
neighbors to minimize complaints.

12. NPDES: NPDES monitoring is to be performed by Preston with copies of the records
maintained on-site and provided to Joe Reynolds. Brian Scott will provide copies of the forms to
Preston.

13. Erosion and Sediment Control Maintenance: Preston’s responsibility. Do not allow anything
into on-site stream. Self-Inspection Forms will need to be completed by Preston documenting
major phases of land disturbing activity. Copies of these forms will be provided to Preston by
the site for their use. Preston is responsible for control of storm water from the existing cells.

14. Stored Materials: Preston to store materials in areas with proper drainage and protection from
weather if needed for specific construction materials. Republic Services will pay for stored
materials on pay requests. Material invoices may be required as backup at Owner’s option.
Location of all stored materials to be coordinated with Joe Reynolds.

15. Time weather delays / work expedition: Preston is to record site rainfall. Consideration for extra
days will only be for rain days beyond the normal rainfall. The rainfall frequency will be used
along with rainfall totals to keep up with rainfall statistics. Unusual circumstances will be
evaluated by HHNT considering rainfall and weather conditions as related to work delays. All
requests for time extensions due to weather should be made immediately following the event and
in accordance with the construction documents. Requests for time extensions submitted at the
end of the job will not be accepted. Preston should plan to suspend time around any holidays
and this should be built into the project schedule.

16. Construction Water: May be obtained from the on-site pond or the well located north of the
project area. Additional water sources are not anticipated. This should be coordinated with Joe
Reynolds.

VI. CONSTRUCTION QUALITY ASSURANCE PLAN:

A. Soils: The CQA testing and sampling will be performed by the CQA representative on behalf of
Republic Services. The borrow source for compacted clay liner will be from Borrow Area No. 3
and/or the Proposed Cell 14B project area. ACC will assist with evaluating the Cell No. 14B area for
possible clay material. Any undercutting required in the cell area will be quantified daily by the site
superintendent and Preston’s CQA Technician. The agreed upon quantity will be recorded in the
daily reports. Alternatively, clay liner material that is over excavated in the cell area may be
surveyed. This will be coordinated between the Owner and Preston after ACC’s Evaluation of
Potential Clay Materials is completed. Unsuitable soils will be evaluated and recorded on a daily
basis by the on-site resident engineering technician and must be approved by the CQA engineer.
Notification of the required geologic subgrade inspection shall be provided to Ms. Christine Ritter.

B. Subgrade Soils: No comment.

C. Conformance Testing:

1. Qualifications and Submittals – The CQA firm is to sample, test and report as described in
the CQA Manual. ACC does plan to use a nuclear gage for this project.

2. CQA Conformance Testing – To be performed by the CQA firm. Third Party conformance
testing for the project geosynthetics will be performed by TRI/Environmental under their
contract with Republic Services under the technical direction of the CQA Engineer.

D. Piping and Liquid Containment:

1. Qualifications and Submittals for the piping are to be provided to HHNT (Matt Cheek, P.E.).
Pressure testing of pipe will not be required. A complete camera inspection and jet-cleaning
of all collection lines will be required. Copies of the inspection shall be included in the final
CQA Report and maintained in the Facility Operating Record. Electro fusion coupling of the
leachate pipe sections is not allowed. These couplings will only be allowed on a case by case
basis if approved by the engineer.

E. HDPE System:

Storage: Preston is responsible for off-loading the geosynthetic materials and for placing them in a
dry, stable area for later use on the project. The location of the lay down area is to be selected in
coordination with Joe Reynolds.

F. Protective cover delivery into the cell requires minimum 4-foot thick haul road in the cell. ACC will
take periodic samples of the material being delivered to the site to ensure conformance with the
project specifications. Any indication that the material being delivered cannot meet the project
specifications should be submitted to HHNT immediately. A 5 gallon sample should be provided to
ACC in conjunction with the material submittal.

VII. CQA ACTIVITIES:

Weekly CQA reports will be prepared and provided by email to Mike Gurley and Matt Cheek, P.E.

In addition, the CQA technician will provide daily updates to Joe Reynolds. However, communication with
the Owner on site related issues will be responsibility of Preston. The CQA report is expected to be submitted
to NCDEQ in one submittal.

VIII. MISCELLANEOUS ITEMS:

1. Road improvements on the access road to Borrow Area No. 3 will be required due to anticipated truck traffic.
(Additional silt fencing may also be required)

2. Interference with landfill traffic is not expected but all construction vehicles should yield right of way to
landfill traffic and/ or local customers.

3. Discrepancies in plans should be brought to HHNT’s attention ASAP.

4. At the completion of construction, all access roads should be dressed up and restored to pre-construction
conditions.
5. Borrow area clean-up will be completed at the end of construction such that all areas remain accessible and
free draining to the existing sediment Basin. There should be no ponding.
6. All efforts should be made throughout construction to be respectful of neighbors and minimize complaints.
Republic will notify all neighbors of the upcoming project.
7. At the completion of the project, sediment Basin 5 Forebay and Borrow Area 3 sediment Basin should be
restored to their Pre-construction condition. Sediment Basin 5 Forebay and Borrow Area 3 sediment basin
conditions should be documented before any construction begins.

8. Contractor should take all necessary precautions related to project safety in and around areas that could
contain methane gas.

9. Republic will move all empty bins and containers from the Contractor staging area.

10. Preston anticipates moving in equipment on 1/30/17.
11. A determination regarding grinding or burning of cleared debris must be approved by the Owner. If burning
is performed, Preston is responsible for obtaining all burn permits. HHNT will notify the Solid Waste Section
and the Air Quality Section prior to initiating burning activities.

12. Payment for grassing shall be 50% when planted and 50% when established.

13. Clay liner tie-in: Top of clay is the control. May be tying into cells with 24” of clay or 18” of clay. New liner
needs to be tied into existing liner at the existing elevations.

14. Preston indicated that they would be looking to hire local folks where possible.

For the discussions in the upcoming pre-operational meeting on this Friday, 08/11/2017, I send you the draft
comments (DIN 28384) on the CQA Report for the construction of Cell 14B (DIN 28372). The comments are
stated below:

1. (Section 4.3 & Section G-4) Please correct the likely typographic errors in Section 4.3:
CL-43R1 should be CL-43R2 and CL-72R1 (a duplicate) should be CL-71R1.

2. (Section 5) Please provide additional info or clarification requested below:

i. (Section 5.1) Please provide a copy of manufacturer’s 20-year material warranty of the 60-mil
HDPE [Technical Specification Section 02750, Paragraph 1.04].
ii. (Section 5.1) Are the constructed geomembranes smooth or textured 60-mil HDPE liners or both
(textured one placed on the sloped areas and smooth one placed on the flat areas)? Please
clarify.
iii. (Sections 5.2 & 5.3) Have the rolls of geotextile been tested and inspected for broken needles by
the manufacturer or the CQA Engineer? Please clarify or provide a manufacturer’s
certification(s).

3. (Section 7) Please provide additional info or clarification requested below:
i. Three options of the protective cover were approved in Technical Specifications for Phase 4
construction (May 2014, DIN 23531), please describe which option is selected and constructed at
Cell 14B.
ii. The stated ASTM testing methods for calcium carbonate content (ASTM D 4373) and
permeability (ASTM D5084) in Section 7.1 are different from those (ASTM D3042 & ASTM
D2434, respectively) used in the laboratory testing presented in Appendix S. Please clarify.
iii. This Section shall report the completed tasks/activities associated with leachate piping
installation according to Technical Specifications – Sections 02745 & 02748 and Rule 15A
NCAC 13B .1624(b)(11)(C). The specified physical properties of the piping material from
manufacturers and/or laboratories should be provided in the CQA Report.

4. (Section 8 and As-built Drawings)
i. Are the grid elevations presented on the Figures 1, 2, & 4 the measurements being perpendicular
to the surfaces – sloped and flat areas as required in the Technical Specification 01050? Please
clarify.
ii. If the answer of the previous comment is “no,” then please re-examine/confirm the survey data
on Figure 2 - Top of Clay; two locations have the measured thicknesses of the in-place
compacted clay liner less than the minimum of 2-feet thickness (perpendicular to the side
slope).

5. (Appendix C) According to Rule 15A NCAC 13B .1621(b)(4), the meeting minutes and sign-in sheet
associated with Cell 14B construction should be appended to this CQA report. The meeting minutes
includes pre-construction, progress, or troubleshooting meetings

6. (Appendix G, Section G-4)
i. The in-situ moisture content (31.6%) of sample CL-1 exceeds the specification [at or above 3% wet
of the optimum moisture content (28.3%)]. The reported difference of moisture of +2.3% is a
typographic. Please make a correction.
ii. The remarks for sample CL-70R1, CL-71R1, and CL-72R1 are likely incorrect. Please clarify.

7. (Appendix G, Section G-5) Please add the sample location CL-13R1 to the drawing.

8. The CQA Report should include the tasks/activities and testing result data stated in the approved work
plan (DIN 27540) and the updated Technical Specification – Section 02220 dated September 2016
regarding construction of an underdrain underneath the Cell 14B. Please provide the requested testing
result data and as-built drawing.

After initial jetcleaning was completed on 7/10/17, the above piping was video-inspected in
entirety using certified explosion-proof equipment (see included CCTV Survey List, Pipe
Graphic Reports, and Videos). Jetclean America was remobilized to the project on 7/20/17 to re-
jetclean and re-video the pipe areas between Rain Flaps A/B and the Tie-Ins to existing Cell
14A. All areas of the piping viewed with the inspection camera appear to be in good condition,
with no defects noted.
