c. Summary:
• Landfill operations contractor was installing a supplemental chimney drain in Cell 1
• Excavation equipment came in contact and damaged the geocomposite drainage
layer (uppermost geosynthetic layer).
• Investigation of the area revealed that the primary geomembrane was undamaged.
d. Field actions:
• The damaged geocomposite was repaired in accordance with the landfill
construction specifications.
• The geocomposite defect was patched using the same type of geocomposite used
during original installation.
• The patch extended beyond the limits of the defect and was secured.

Dear Ms. Martin:
On July 20, 2017 an approximate 2-ft x 2-ft section of the Dan River Landfill Cell 1 leachate
collection geocomposite drainage layer (uppermost geosynthetic layer) in the vicinity of the Cell
1 intercell berm was damaged by excavation equipment during installation of the Cell 1
supplemental chimney drains. Amec Foster Wheeler construction quality assurance (CQA) and
Duke Energy personnel observed the damaged geocomposite and underlying primary
geomembrane. The underlying primary geomembrane was observed to be undamaged.
The damaged geocomposite was repaired by Texas Environmental Plastics (TEP) on July 20,
2017 in general accordance with the landfill construction specifications. The geocomposite
defect was patched using the same type of geocomposite that was installed during Landfill Cell
1 construction. The patch extended beyond the limits of the defect and was secured to the
undamaged geocomposite using zip ties. A geotextile patch was heat sealed over the extents of
the geocomposite patch. A field report by Amec Foster Wheeler is provided as an attachment
documenting field activities for the repair of the geocomposite.
