The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the
Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty
of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of
Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties,
injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve
compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The Septage Land Application Site consists of Field 1 (21.4 acres), Field 2 (14.5 acres), Field 3 (21.5 acres) and Field 4 (7.4
acres) with a total of 64.8 acres.
2. The fields are permitted for land application of domestic septage and grease septage.
3. The site had accessible all-weather roads to the septage land application fields covered under the permit.
4. Field 4 had cattle and not currently used for land application. Fields 1 and 2 were not being used. Field 3 was being used for
recent land applications. As a reminder,
5. Appropriate signage was displayed.
6. Disposal area boundaries were delineated with permanent markers. Addition markers have been placed to account for the
cutting of the forest and replacement of those that have fallen or are missing. More markers are to be placed.
7. All septage discharges appear to have been made within the disposal area boundary with the small exception of the eastern
boundary of Field 1. As a reminder, all septage discharges must be made within the field boundaries.
8. In Field 3 there were spots at the beginning of the application were heavy.
9. No objectionable odors were observed.
10. A water supply well was observed within 500 feet of Field 2. Per the Burke County Health Department, this well has been in
place since at least 2008. The boundary of Field will need to be reviewed during the next permit revision.
