Report #2 – June 26, 2017 ReUse Technology, Inc. Former Swift Creek Coal Combustion By Product (CCB) Structural Fill, CCB0057 Battleboro, North Carolina Introduction   ERM NC, Inc. (ERM) has been contracted by ReUse Technology, Inc. (ReUse) to implement the Groundwater Corrective Action Plan for the Former Swift Creek Coal Combustion By Product (CCB) Structural Fill, CCB0057 approved by NCDEQ – Division of Waste Management on March 12, 2015. As requested by NCDEQ, ERM will provide monthly updates to summarize site activities including any changes in conditions as the project progresses. ERM has subcontracted Earnhardt Grading, Inc. (Earnhardt) of Lowesville, North Carolina to provide site preparation, excavation, grading, geosynthetic installation and final site stabilization. ERM and Earnhardt have been on the Swift Creek site full time beginning April 24, 2017 for construction activities. ERM has compiled weekly update status reports that are attached for reference. Summary of Activities Since the previously issued Monthly Project Update dated June 24, 2017, ERM and Earnhardt Grading have remained onsite full time conducting corrective action plan activities. These activities included:  Removal of CCB materials from the south end (Phase 1 area) of the project site.  Stockpiling of CCB materials on the north end (Phase 2) area of the project site.  Maintaining erosion and sedimentation control measures per NCDEMLR – Division of Land Quality approved plans (approved March 31, 2017).  Excavation and removal of previously abandoned 24-inch reinforced concrete storm pipe (partially complete).  Maintaining previously stockpiled cover soils area at the south end of the project site. At the end of this second month of construction activities, Earnhardt has completed CCB material removal of approximately 60 percent of the Phase 1 footprint area. Due to heavy rains in the vicinity of the project, there have been several days lost to production due to weather and/or wet conditions. ERM NC, Inc. 15720 Brixham Hill Avenue Suite 120 Charlotte, NC 28277 (704) 541-8345 (704) 541-8416 (fax) Swift Creek Monthly Update Report #2 Page 2 June 26, 2017 Site Observations  To date there have been no visual indications of dust migration off site.  Since arriving on site, approximately 21-inches of rain has been recorded based on the onsite rain gage and monitoring.  Erosion and sedimentation control measures are functioning appropriately and there have been no observations of offsite sediment releases.  Monitoring wells have been marked with construction fence and maintained throughout construction activities.  The abandoned 24-inch diameter reinforced concrete storm pipe located in the Phase 1 area of the project site was encountered on 6/8/17 while performing CCB material removal along the southeastern portion of the site. At this location the pipe was open and contained water; not full of grout as previously reported by the site owner. The pipe continued open to the east approximately 60 feet from point of discovery and the outlet end was observed as submerged in the “swamp area”. This section of pipe was removed from the outlet to the point of discovery within the Phase 1 area. Following removal, this area was backfilled with onsite soils to re-establish runoff containment within Phase 1 of the project. As removal of ash continues from south to north, the pipe will be evaluated for containing grout or open. If the pipe is open, pipe will be removed and transported to the ash stockpile area. If the pipe is found to be filled with grout and appears structurally sound, it may remain in place. The locations of pipe, pipe abandonment and results of observations will be recorded. Reference the attached Weekly Report dated June 14, 2017 for additional information.  ERM discussed the discovery of the pipe and conditions with Jackie Drummond of NCDEQ on June 12, 2017. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified that erosion and sedimentation control measures are in place for an onsite meeting. Thad replied that he would stop by next time he was in the area. No visit has occurred yet. 2. NCDEQ – Waste Management – Reached out to Jackie Drummond to ask if the Division had decided on a date to visit the site. Jackie responded that it would likely be sometime in July. Meeting has not been scheduled yet. Project Related 3. Earnhardt Grading and ERM remain on site full time since 4/24/17. 4. Earnhardt has finished removal of topsoil and cover materials minus one path for access on the west side of the site. 5. Excavation of ash materials has begun on the south end of the site with transportation to the north end; excavation of Phase 1 area. The bottom of the beneficial fill has been identified in the south end of the project (see photos). 6. Thus far for the week the site is dry and Earnhardt was able to excavate and transport 202 loads of ash materials on Tuesday May 30th. 7. Photo Log of activities is attached for reference. Swift Creek Project Weekly Update – June 7, 2017 Key Items Completed Last Week: Permit Related 1. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified that erosion and sedimentation control measures are in place for an onsite meeting. Thad replied that he would stop by next time he was in the area. No visit has occurred yet. 2. NCDEQ – Waste Management – Reached out to Jackie Drummond to ask if the Division had decided on a date to visit the site. Jackie responded that it would likely be sometime in July. Meeting has not been scheduled yet. Project Related 3. Earnhardt Grading and ERM remain on site full time since 4/24/17. 4. Excavation of ash materials continues on the south end of the site with transportation to the north end; excavation of Phase 1 area. The bottom of the beneficial fill has been identified in the south end of the project (see photos). 5. This week the site is wet causing slow work Monday through Tuesday. Earnhardt was able to work Sunday. To date Earnhardt has reported moving approximately 15,000 cubic yards. Progress Plan attached. 6. Tom Sears with Full Circle visited the site the morning of 6/7/17 and toured the progress with Chris Means of ERM. Chris reports that Tom seemed pleased with the progress and site conditions. 7. Swift Creek Project Weekly Update – June 14, 2017 Key Items Completed Last Week: Permit Related 1. Thad replied that he would stop by next time he was in the area. No visit has occurred yet. 2. NCDEQ – Waste Management a. On site meeting with NCDEQ is scheduled for August 8, 2017 at 9:00 am. b. ERM updated Jackie Drummond via phone call on June 8, 2017 concerning open pipe discovered in lieu of abandoned pipe. Jackie requested no additional information and was appreciative of the update. ERM reported to Jackie that the open pipe was removed and berm area was backfilled for containment. ERM also added that the pipe will be discussed in our next monthly update report. Project Related 3. Earnhardt Grading and ERM remain on site full time since 4/24/17. 4. Excavation of ash materials continues on the south end of the site with transportation to the north end; excavation of Phase 1 area. The bottom of the beneficial fill continues to be identified in the south end of the project (see photos). 5. This week the site has been dry allowing for good production. Phase 1 ash removal is approximately 36% complete. Progress Plan attached. 6. Open reinforced concrete pipe discovered along east side of fill area approximately 30-ft north of monitoring wells MW-1S and MW-1D. This pipe was reportedly abandoned with grout according to Full Circle. Tom Sears with Full Circle was notified of the open pipe discovery on Friday June 9th. Tom indicated that the pipe was abandoned from the west end near Highway 301 by filling with grout. It appears that grout never reached the east end of the pipe run. ERM developed a Technical Memorandum on June 9th and distributed to Client Team. To date, Earnhardt has removed approximately 64 feet of the reinforced concrete pipe working from the east end outlet west into the fill area. All pipe removed has been observed as open. East berm was backfilled with soil following pipe removal to maintain containment. 7. Water gathered in the area of the open pipe discovery was pumped to two (2) sprinklers located on the top of the fill area to limit the amount of “ash contact water” reaching the east basin and to also assist with dust control. Following pumping the water level did not recover to original elevation upon discovery.   8. On June 12th, Earnhardt along with ERM pumped water into the storm water catch basin along US 301 using the onsite water wagon and monitored the east end of the open pipe to observe indications of increased flow. There was no increased flow at the east end observed. Therefore, the reinforced concrete pipe may still be adequately abandoned at some location within the fill area. 9. On the morning of June 12th, a vehicle accident occurred in front of the project site entrance and office trailer location involving a State Trooper patrol car losing control during a high speed pursuit and landing in the north bound roadside ditch within the US highway 301 right of way. There was no property damage to the project site as the incident was confined to the right of way limits. ERM observed orange conduit piping exposed following removal of the car from the ditch area that may be associated with the fiber optic communication line along 301. 10. Continue excavating ash working routinely from south to north for preparation of Phase 1 area. Anticipate completing Phase 1 removal within the next two weeks. 14. An ERM onsite health and safety audit is scheduled for June 21st led by ERM’s Health and Safety Director for the South Atlantic region. 15. ERM will continue to update Full Circle concerning site activities as needed. Schedule Original schedule submitted by Earnhardt completes removal of CCR materials from Phase 1 area 10 weeks from beginning mobilization. Currently at 7-1/2 weeks Phase 1 removal is 36% complete with good production each day. With good workable weather the project should remain on schedule.               ReUse Technology, Inc. Swift Creek Project Weekly Update – June 21, 2017 Key Items Completed Last Week: Permit Related 1. NCDEQ; DEMLR – Land Quality – Thad Valentine was notified on May 12, 2017 that erosion and sedimentation control measures are in place for an onsite meeting. Thad replied that he would stop by next time he was in the area. No visit has occurred yet. 2. NCDEQ – Waste Management a. On site meeting with NCDEQ is scheduled for August 8, 2017 at 9:00 am. Project Related 3. Earnhardt Grading and ERM remain on site full time since 4/24/17. 4. Excavation of ash materials continues on the south end of the site with transportation to the north end; excavation of Phase 1 area. The bottom of the beneficial fill continues to be identified in the south end of the project (see photos). 5. ERM is conducting a health and safety audit at the site on 6/21/17 with ERM’s South Atlantic Health & Safety Director Millard Griffin along with Earnhardt Grading’s Health & Safety Director Anthony Dohmann. 6. Former temporary ash stockpile area located near the center of the site was removed and transported to the north ash stockpile area. 7. This week the site has had over an inch of rain slowing production. Phase 1 ash removal is approximately 60% complete. Progress Plan attached. 8. Due to rain, no work on site Tuesday June 20th. Continue excavating ash working routinely from south to north for preparation of Phase 1 area. Anticipate completing Phase 1 removal within the next two weeks. 12. Currently discussing an alternate approach including raising the first half of the Phase 1 floor to receive ash materials as the north stockpile is becoming difficult to manage due to wet conditions. The floor would be raised using on site soils, compacted and surveyed prior to receiving ash materials. 13. ERM will continue to update Full Circle concerning site activities as needed.   14. Earnhardt is discussing work schedule around July 4th holiday and will update. 15. ERM will prepare the second monthly update for NCDEQ for submittal on June 26th. Schedule Schedule submitted by Earnhardt completes removal of CCR materials from Phase 1 area 10 weeks from beginning mobilization. Currently at 8-1/2 weeks Phase 1 removal is 60% complete with production delayed by rain. A 40 – 60 percent chance of rain remains in the forecast for the next several days.
