Dear Mr. Sparks:
Please find enclosed the signed Permit to Operate (DIN 25144) for the Dixie Classic
Fairgrounds Facility located in Winston-Salem, NC. This permit issuance approves the
continued operation of the facility under the terms and conditions of the permit. In
general, the facility permit does not reiterate permit conditions included in the Solid Waste
Management Rules, 15A NCAC 13B, nor does it restate specific conditions that are
included within the approved plans, such as site specific operations. Please review the
conditions of the permit carefully.

Please keep a copy of the current permit, updated Operations Plan, and other required
documentation on site at all times. These are also available for download from our
