Dear Mr. Eller, 

The Solid Waste Section (Section) has reviewed the Well Abandonment & Well Replacement 
Request (DIN 27839) submitted on your behalf by S&ME, Inc. on May 24, 2017.  The request 
outlined the abandonment of MW‐1, MW‐8, MW‐10 and MW‐12 and the replacement of MW‐
2B.  The Section approves this request. 

After the installation of MW‐2C, submit the boring log and well construction record. 
Groundwater wells MW‐1, MW‐8, MW‐10 and MW‐1 shall be properly abandoned by in 
accordance with 15A NCAC 2C .0113, entitled “Abandonment of Wells”.  Well abandonment 
records (Form GW‐30) well must be certified by a Licensed Geologist in accordance with rule 
.1623(b)(2)(1) and submitted to the Solid Waste Section in accordance with 15A NCAC 02C 
.0114(b).
