STATUS OF PAST NOTED VIOLATIONS:
No violations noticed on the previous inspection conducted on 05/11/16.

OBSERVED VIOLATIONS:
NONE

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present at you get to the site. Need to cut vegetation so that it does not block the sign. See
picture below.
2) Access road well maintained.
3) Markers present and visible.
4) No septage discharges noticed outside of the permitted boundary.
5) No application issues noted. No spot dumps or thick applications observed. Septage had been
applied to the site today.
6) The bermudagrass needs improvement. Weed control needed. A lot of nutgrass noticed. See
picture below.
7) pH of septage not checked. No application at time of inspection.
8) No objectionable odors noticed today.
9) No change noticed with site. Setbacks ok.
10) Records not reviewed.
11) I collected a composite soil sample from each field. The site has two fields.
