OBSERVED VIOLATIONS: N/A SEE BACK The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected the land application site. The required signs are posted and the access road is adequate. It appears that approved setbacks are being met but were not measured at this time. The field markers appear adequate. No sign of land application outside the permitted boundaries. No sign of recent or heavy application. No odors noted. The crop stand appears adequate at this time. Maintain cropstand and control weeds as needed. Discussed that in the future soil sampling will be handled by the permit holder, not the Solid Waste Section. 2. Discussed the permit modification submittal. Discussed the SLAS application and minor changes that were needed. Submit a corrected SLAS application with current signatures. Discussed what fit the definition of industrial/commercial septage and that this type of waste cannot be land applied without prior approval. Discussed that the property is currently owned by A Great Company LLC and that a new landowner authorization form is needed. A nutrient management plan with current signatures is also needed. Logs were submitted for review. The test for the land residuals class (in regards to the higher application rate at this site) will be in June. 3. Mail all hard copies of above requested items.
