On December 7, 2016, the Solid Waste Section (Section) on NCDEQ Division of Waste Management (DWM)
approved the Proposed Groundwater Assessment and Monitoring Plan (DIN 27003) for the structural fill at the
Duke Energy Belews Creek Steam Station. That plan was divided into two phase which included the
installation of one well cluster (one shallow, one deep) during Phase 1 and four additional well clusters during
Phase 2. The Phase 1 wells have been installed.

Upon further evaluation, it has been determined that there phasing of the installation of wells is not
necessary. Preparations are being made to install the remaining four well clusters.
Duke Energy requests concurrence from DWM to install the remaining well clusters and conduct the sampling
of all wells upon well installation.
