FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. McPherson Septic Tank Cleaning under Steve McPherson, NCS-00306 has been sold to Brian Paris with the same firm name (McPherson Septic Tank Cleaning) under the new permit number NCS-01353. Inspected the vehicle, tag#YA017853, 3000 gallons, VIN1NKDLU0X045866332. 2. The tank appears structurally sound and securely attached. 3. Outlet covers were on and no drips/leaks were noted. 4. The lettering is correct on both sides of the vehicle. However the decals for the firm name are somewhat damaged (on the passenger side) and will need replaced in the near future – still legible at this time. Also the phone number displayed on the vehicle is the business number (336-226-1918) – please list this phone number on future application forms. 5. I informed Corinne Law in the Raleigh Central office that the vehicle was in compliance in regards to issuing the new permit. Mr. Paris indicated that he had a copy of the example truck pumping log to begin using once the firm permit was issued and septage jobs could begin.
