The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle , tag#HF3722, VIN 1FDAF56F11ED62785, 500 gallons. The tank is structurally sound
and securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both
sides of this vehicle. This vehicle is in compliance at this time.
2. Also inspected vehicle tag#EF7215, 800 gallons, VIN3FRWF65H76V339822 which is different than the VIN
listed on the recently submitted firm application. Please ensure that the correct tag#, VIN & tank capacity
are listed on the firm applications each year for permit renewal. The tank is structurally sound and securely
attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of this
vehicle. This vehicle is in compliance at this time.
3. Did not inspect the other two permitted vehicles (tag#EF7211 & DF7356) at this time. Records indicate that
they were inspected in 2015.
4. Truck logs were not inspected during this inspection. Please email me a few pages of the truck logs for
review to ensure that the required information is being maintained.
5. Upon review of the file & recent permits I noted that the office location (town name) changed from Siler City to
Bear Creek at the end of 2015, however the mailing address town name is still Siler City.
