Dear Ms. Ritter:

Smith Gardner, Inc. (S+G) recently oversaw groundwater monitoring well and landfill gas
probe (LFG) installation for locations related to the updated Groundwater1and LFG2
Monitoring Networks at the Johnston County Landfill Facility (SWS Permit No. 51-03) in
Smithfield, North Carolina. S+G personnel met with drillers from Geologic Explorations Inc.3
March 21-23, 2016 for well and probe advancement activities. The following summarizes
those field activities.

Monitoring Well Installation

Per the approved Water Quality Monitoring Plan, one groundwater monitoring well (PM-1)
was installed as a replacement detection monitoring well for cross-gradient wells CDMW-2,
MW-4B and PZ-3. The existing wells (CDMW-2, MW-4B and PZ-3) were located immediately
adjacent to waste and therefore samples were not representative of aquifer conditions near
the compliance boundary.

PM-1 was constructed east of closed MSW Phases 1 and 2 in accordance with 2C regulations.
PM-1 was advanced to 15 ft. below ground surface (bgs) using 4.25 inch hollow stem augers
(HSA). The well was finished with a 3 ft. stickup, steel above-grade casing and lock. The
approximate location PM-1 is shown on Figure 1. To date this well has not been surveyed
but will be in the near future. The well construction record and boring log are presented in
Attachment A.

LFG Probe Installation

Four LFG probes (LFG-7A, LFG-8A, LFG-10A and LFG-13) were installed as part of the
updated monitoring network.

LFG-7A: Installed as a replacement for irreparably damaged LFG-7. LFG-7A was installed
to a depth of 7 ft. bgs.

LFG-8A: A replacement probe for LFG 8 was installed at the new facility boundary to ensure
area coverage. LFG-8A was installed to a depth of 6 ft. bgs and is located across the
headwaters of Buzzard Branch.

LFG-10A: Installed closer to the revised facility boundary as a replacement for LFG-10,
which had consistent detections. LFG-10A was installed to a depth of 13 ft. bgs.

LFG-13: Installed to monitor the southern property line south of Phase 5. LFG-13 was
installed to a depth of 18 ft. bgs.

LFG probes were installed in accordance with NCAC 2C regulations. All four locations were
installed with 4.25 inch HSA and finished with approximately a 3 ft. stickup, well cap with a
quick connect fitting, steel above-grade casing and lock. Approximate LFG probe locations
are shown on Figure 1. Construction records and boring logs are presented in Attachment
B.
