No violations noted on the previous inspection conducted on 07/27/16.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Received application for modification of the storage capacity of the facility. Some tanks have been removed
from being used due to previous leaks or the tank wall getting thin. I inspected six (6) steel tanks that had been
added to the facility. Seven (7) steel tanks remained from the current permit. The additional 6 tanks bring the
total tank number to 13. No issues noted with the 6 added tanks. See the attached page for tank size.

2) Tanks accessible for inspection.
3) Tank #3 appears to have some holes. See pictures for location. Tank #3 is one of the previous tanks.
Possible holes at both sides of Tank #3 in the area where
the arrows are pointing.
Possible holes at the side closest to Tank #4 at the
opposite end.

4) No septage spills noticed on the tanks or on the ground by the tanks.
5) No odor or vector issues noticed today.
6) No change noticed that would affect setback distances.
7) “No Trespassing” sign present with the Septage Land Application Site sign along the access road before you get
to the site.
8) Be sure that any septage within the discarded tanks is removed and not just left within the tanks. Tanks have
been placed next to the woods.
9) No issues noticed with the six (6) added tanks. The added tanks can be used for storage of septage once the
permit has been issued. Permit issued on 10/07/16.
