No violations noted on the previous inspection conducted on 05/10/16.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present on both sides of the path as you get to the site. Signs need to be repainted.
2) Access road is well maintained.
3) Disposal boundaries for both fields clearly marked. Markers visible.
4) All septage discharges within the permitted boundaries of the fields.
5) No spot dumps or thick applications noticed today.
6) The small grain needs to be planted onto both fields. Broadleaf weed control is also needed.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed.
9) No change noticed that would affect setback distances.
10) Application for permit renewal received on 09/09/15. Records submitted with application.
11) I collected a composite soil sample from each field. A total of 2 composite soil samples taken.
