with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present on both sides of the path as you get to the site. The lettering on the signs is fading. Please
repaint the letters.
2) Access road is well maintained.
3) Markers were present and easily visible.
4) All septage discharges within the permitted boundaries of the fields.
5) No spot dumps or thick applications noticed today. Some ruts observed to the southern side of Field A. See
picture below.
6) The ryegrass needs to be harvested off of both fields.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed.
9) No change noticed that would affect setback distances.
10) Application for permit renewal received on 09/09/15. Records submitted with application.

Ruts observed in Field A. Ryegrass present on both fields. Need to be harvested.
