No violations noted on the previous inspection conducted on 10/20/16
OBSERVED VIOLATIONS:

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) "No Trespassing" sign present as you get to the land application site.
2) Inspected four metal tanks. Tanks accessible for inspection.
3) The first tank from the left has a hole, about ¼ inch in diameter, near the bottom of the front end. Also, there
appears to be a leak starting along the middle edge of the last tank front the left. See pictures below. No septage
leaking from the holes today. Holes need to be patched.
4) Septage transfers to and from the tanks appear to be handled in a safe and sanitary manner. No septage spills
noticed around the tanks.
5) No objectionable odor or vector issues noticed today.
6) No change noticed that would affect setback distances.
7) The tanks appear to be used on a regular basis. It does not appear that the first tank is being used.
Hole present in the first tank from the left. Potential hole in the fourth (last) tank from the left.
