Mr. Creighton,

The Division of Waste Management, Solid Waste Section (Section) has completed a review of
the submittals titled Permit Modification, Phase 5B (DIN 27689) and Construction Quality
Assurance Plan, Phase 5B (DIN 27727). The documents were submitted to the Section on
your behalf by McGill Associates and received in the Asheville Regional Office on May 3, 2017
and May 11, 2017, respectively.

Based on this review, the Section approves the soil base installation in the approximately
0.2‐acre Phase 5B area of the C&D unit. The soil base must be installed, tested and
documented in accordance with the following plans:
1. Permit Modification, Phase 5B (DIN 27689),
2. Construction Quality Assurance Plan (DIN 24948, pp. 529-541)
3. Construction Quality Assurance Plan, Phase 5B (DIN 27727)

Waste filling activities in the Phase 5B area may only commence with written authorization
from the Section after receipt and review of the Construction Quality Assurance Certification
Report.
