Permit renewal in process per Donna Wilson – NCDEQ – DWM - Permitting PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS; 01 - Permit to Operate, Compost Type 2. A Change of Ownership was done on August 12, 2015 and permit expired on January 12, 2016. Renewal Application has been sent to DWM Permitting. John College while on-site, called and spoke with Donna Wilson, DWM permitting. Permit renewal is in process. 02 - The facility is well secured by gates fences and berms. The site has signs with all required information displayed. 03 - This facility has a Plant Manager (Julius D. Leary) who is onsite during operating hours that are Monday – Friday, 7:30am to 4:00pm. 04 - Interior roadways appear to be in good condition. Per our conversation onsite today, you do need to make the access road on the North side of the yard drivable to ensure vehicular access available in the event of a fire and for leachate monitoring. Continue efforts (as weather allows) to get road finished on North Side. 05 - Materials stockpiled onsite (pine bark, peanut hulls and cotton gin trash) were all the approved type for this site. 06 - The total capacity for this site is 80,000 cubic yards. Processed and stored. 07 - Windrow dimensions appear to be in compliance. Maximum dimensions10 feet high and 22 feet wide. 08 - Feedstock, curing and finished product stockpiles appear to be in compliance. Limited to 30 feet high and 50 feet wide. 09 - Temperatures are being monitored. Be sure to have at least 131 degrees for 3 consecutive days and have your thermometers calibrated annually. 10 – This facility is closed to the public. 11 – Facility has Analytical Test done at Waters Agriculture Lab, dated 09-02-2016 on finished product and has sent them to DWM in Raleigh. 12 – Digital pictures taken on this facility inspection.
