Dear Mr. Moriarty:

The Division Waste Management (DWM), Solid Waste Section (SWS) completed reviews of the
following permit application document:
 Operation Plan for Overdale Road Transfer Station, Winston-Salem, North Carolina. Prepared
by Hodges, Harbin, Newberry & Tribble, Inc. (HHNT). Dated March 13, 2017 and revised April
19, 2017. (DIN 27663).
Based on the reviews, the SWS has no further comment on the above-referenced permit
application document at this time and, hereby, issues Republic Services of North Carolina LLC.,
a wholly owned subsidiary of Republic Services, Inc. a Life-of-Site Permit Approval to Operate
(DIN 27670) to continue operating the Overdale Road Transfer Station, Solid Waste Permit No.
3416T-Transfer- according to North Carolina General Statute (N.C.G.S.) 130A-294.(a2). This
permit shall not expire until the SWS approves final closure of the facility or revokes the permit
according to North Carolina Solid Waste Management Rule 15A NCAC 13B .0703 and N.C.G.S.
130A-23.

Enclosed please find a hard copy of the permit (DIN 27670) and a certified copy of the permit
for the recordation purpose. Please pay special attention to the General Permit Condition No. 3
of Attachment 1 of the permit regarding permit recordation requirement. The certified copy of the
permit (enclosed) must be filed in the Register of Deeds office in Forsyth County, and the copy
(entire document including permit) is returned to the SWS, within thirty (30) calendar days, from the
date received, with the page and book number, date of recordation, and Register’s seal.
