No violations noted from the inspection conducted on 08/03/15.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Required signage present.
2) Inspected six (6) steel tanks. Tanks accessible for inspection.
3) Only five (5) tanks are usable. The first tank is unusable and has do not use written on it. The other tanks
appear to be fine. No leaks noticed.
4) A little bit of septage on the ground by the tanks. Need to be careful when transferring septage to and from
the tanks.
5) No objectionable odors or vector issues noticed today.
6) Setbacks appear to be ok.
7) Tanks appear to be used on a regular basis.
