On November 23, 2016 the Division of Waste Management, Solid Waste Section (Section) received
Charah, Inc.’s construction quality assurance (CQA) documentation for Subcell 2B of Permit No. 1910-
STRUC-2015, the Brickhaven No.2 Mine Tract “A” Mine Structural Fill (Facility), entitled;
Construction Certification Report, Brickhaven No.2 Mine Tract “A” Structural Fill – Cell 2, Subcell 2B,
Permit 1910-STRUCT-2015, Charah, Inc., Moncure, North Carolina, November 23, 2016. Prepared for
Charah. Prepared by HDR Engineering. November 2016. DIN 27110.
The Section has reviewed the documentation submitted on behalf of Charah, Inc. and hereby approves the
commencement of structural fill operations in Subcells 2B at the Facility. Please note the following
requirements for future construction and operation of the Facility;
 Document ID Number 27110 is considered an approved documents of the Facility’s permit and
will be treated as such. The next time the Facility’s permit is either amended or modified these
documents will be added to the list of approved documents within the permit.
 Charah, Inc. must submit construction quality assurance documentation, for all future
construction and receive approval from the Section prior to placing those areas into operation.
