The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The cropstand appears adequate. Field needs cut soon (after the required 30 day wait).
2. The required signs are posted and the access road is adequate. Received key for gate lock.
3. No sign of heavy or recent application. It appears that the approved setbacks are being met. No sign of land
application outside the permitted boundaries. No odors noted.
4. Reviewed the permit renewal submittal with Emily Cooper. Discussed waste types, septage, sewage,
industrial/commercial. Reviewed with Ms. Cooper the additional information that is needed in the nutrient
management plan to go to a two field rotation. Site logs are under review. Please submit logs for any
discharge events that Hunt’s Septic performed from October 8th (Hurricane Matthew) until now at this
site. The Raleigh office did receive an authorization form for Gavin Hunt to discharge on this site in regards to
his 2017 firm permit.
5. Site shall be divided into two fields for the new permit. Mr. Cooper posted additional posts along the front of
the site in regards to the required 500’ setback from the trailer and from the church buildings. I measured these
setbacks and posted orange flags at midway points to meet the required 500’ radius. I informed Ms.
Cooper by phone of the additional orange flags which needed to be replaced with permanent markers.
Site was GPS’s for the acreage for each field & in regards to a new site map. The previous field markers
on site appeared to be accurate. Site will continue to be used as one field until the new permit is issued.
6. Continue to work with Ms. Georgia Love, regional agronomist, in regards to maintaining the cropstand
& controlling weeds on both of your permited land application sites.
