Hilltop Road Red Springs, NC 28377 PARTICIPANTS: Connie Wylie STATUS OF PERMIT: Active – permit expired July 30, 2016 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A SEE BACK The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The tank appears structurally sound. Appears empty. 3. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No sign of leaks or spills. No vectors or odors noted. 4. It appears that the approved setbacks are being met. No physical change in facility. 5. The required 5’ cleared radius (free of excess vegetation) is present at this time. Please maintain in the coming months. 6. Reviewed the permit renewal submittal with Emily Cooper. There are currently no logs for this detention facility because no other firm has discharged into this storage tank. Currently the Raleigh Central office does not have an authorization form for any firm (other than Cooper Septic) to discharge into this tank – if you wish to allow another firm to utilize this storage tank then an authorization form shall be submitted asap.
