ADDITIONAL COMMENTS
1. Inspected second vehicle to be added to firm permit. Per Mary Johnson the Raleigh central office has received
the $250 check for this second vehicle. Tag#HL-1408, VIN1NAMEO8J0NGF63143, 300 gallon capacity.
2. Tank appears structurally sound & securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on both sides of this vehicle, nothing less than three inches. However please ensure that
all firm applications list the phone number that is displayed on the vehicle.
5. This vehicle may be used for septage pump jobs.
