Dear Gentlemen:

The Division Waste Management (DWM), Solid Waste Section (SWS) has completed reviews of
the certified Construction Quality Assurance Report (CQA Report) for the constructed Great Oak
Landfill – Phase 1, Cell 1B. The referenced CQA Report is titled as:

 Construction Certification Report, Cell 1B, Great Oak Landfill (Permit No. 7607-
MSWLF-2015). Dated March 07, 2017 and revised through April 06, 2017. Prepared by
SCS Engineers, PC in North Carolina. (DIN 27611).

Based on the reviews, the SWS determines that the constructed Great Oak Landfill – Phase 1,
Cell 1B has met the pre-operative conditions stated in the Permit to Construct of the landfill
dated January 19, 2016 (DIN 25142), the approved CQA Plan, the amendments, and technical
specifications and hereby, issues Randolph County (Owner) and Waste Management of
Carolinas, Inc. (Operator) the Permit Approval to Operate – Phase 1, Cells 1A & 1B of the Great
Oak Landfill, Permit No. 7607-MSWLF-2015 (DIN 27612). Enclose please find a copy of the
permit and a copy of the cover pages of the CQA Report with an approval seal. Please place the
approved document and this permit to the landfill facility operating record.
