or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle for the issuance of the first firm permit. Tag#YA137631, VIN 1HTSDN2N7NH453966, 2300
gallons. Tank appears structurally sound & securely attached.
2. The lettering is correct on both sides of this vehicle.
3. Outlet covers were on and no drips/leaks were noted.
4. This vehicle is in compliance at this time.
5. Received a WWTP authorization which has been sent to the Raleigh Central office.
6. Gave Mr. Dodge an example truck log to record the required information for pump jobs – which may begin
once the firm permit has been issued.
7. Enclosed is information regarding a septage land application site permit.
