The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Currently, the Septage Land Application Site consists of Field A.1 (8.5 acres), Field A.2 (10 acres), Field B.1 (12
acres) and Field B (9.5 Acres) for a total of 40 acres. The facility is in the process of reactivation of the site. All
of the fields were re-measured using a Trimble GPS taking into account new wells that have been drilled on the property and a new house. The acreage will change with the new permit.
2. Field A1 was in pasture with cattle with setback adjusted to account for new wells and property boundary. Field A2 is a hayfield and with field boundaries adjusted for removal of a mobile home and property boundary. Field
B1 is a hay field with the usable boundary greatly adjusted for a new house and wells. Field B2 was premeasured
and the adjusted as needed. When necessary, the new boundary was marked with orange flags with many re-placed with permanent markers by Phillip Harris during the inspection and painted them yellow. All orange flags
will need to be replaced with permanent markers.
3. The site had accessible all-weather roads to the septage land application field covered under the permit.
4. No evidence of recent septage disposal was observed.
5. The fields cannot be used until the new permit is issued.
