The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The required signs are posted and the access road is adequate.
2. No recent or heavy applications noted. No apparent odors.
3. It appears that no land application is occurring outside the permitted boundaries. Field markers appear accurate.
However did not drive over the entire site due to recent rains and setbacks were not measured at this time.
4. Please ensure all setbacks are maintained and that field markers are present. Field 2 has a 500’ setback to the
house on the west side, a 450’ setback to the well on the south east side and a 50’ setback to the property lines
on both the south and east sides. Field 1 has a 50’ setback to the property lines on the east and west sides. Field
1 also has a 100’ setback to the trailer on the north side that was previously occupied by Wallace Moore,
one of the permit holders. The Solid Waste Section shall determine how to address the appropriate
setback requirements at this location in the future.
5. Field 1 appeared to have significant patches of vegetation other than the approved crop stand (Bermuda/rye) as
required by the nutrient management plan. Contact your local extension agent or regional agronomist for
specific advice on how to improve/maintain the approved crop and in particular, control weeds.
