The purpose of this letter is to confirm that Stericycle Environmental Solution, Inc. met the pre-operation
conditions of the permit to operate issued December 7, 2016 (DIN 26288) and to serve as written
permission to operate the facility in accordance with Attachment 3 (permit to operate) of the permit.

Attachment 3, Part II, condition 2 of the permit required a site facility inspection by a representative of
the Section prior to operation. The inspection was conducted on February 23, 2017, by Ms. Susan Heim.
