The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Facility is a closed lined MSW landfill, that entered the post-closure period on October 29, 2004.
2. An operator is on duty at the facility during regular business hours to monitor and maintain the gas-to-energy
system. This system was in operation at the time of this inspection
3. Copies of the quarterly methane monitoring reports from 2012 through 2016 were received by the Solid Waste
Section on February 17, 2017 via email, as requested in the previous inspection report.
4. The area of slope failure seen during the previous inspection was observed again during this inspection. Mr.
Lacsamana explained that the problem had arisen over the winter of 2015-16, and that repairs had not been resulted
in the establishment of vegetation that would prevent further slope failure. He added that this area would be
addressed again in April, when annual repair work will be undertaken.
5. Mr. Lacsamana stated that he schedules inspections of the landfill quarterly.
6. Several groundwater monitoring wells and landfill gas monitoring wells were observed during the inspection. All
were properly tagged, labeled and locked. Mr. Lacsamana stated that new labels and tags had been ordered and
applied to the wells.
7. Several landfill gas extraction wells were also observed during this inspection. At the LFG extraction well labeled
#21, the sound of rushing liquid could be heard. Mr. Lacsamana explained that this was likely due to a vacuum
caused by the drop of condensate in the line. He added that the telemetry system provides notice of any irregularity
in the system, and stated he felt confident that this noise did not represent a problem.
8. The five above-ground leachate storage tanks, located on the south side of Freeman Road, are housed within a
small fenced compound that includes a secondary containment area constructed of concrete block approximately
two feet high. The tanks were inspected again to address several areas of concern noted in the previous inspection
report:
a. Some liquid had ponded on the floor of the containment area at the base of one of the tanks. Mr.
Lacsamana stated that there’d been no evidence of a leachate leak, and that the water was the result of
recent rains.
b. Significant rust was observed on the exterior walls of the tanks; areas of staining were visible that appeared
to be the result of water flowing downward from pipes and couplings. Mr. Lacsamana explained that the
tanks are not made of metal, so the discoloration was not due to rust. He stated that the telemetry system
indicated no leaking and that the pipes and connectors were all inspected regularly, as was the rest of the
leachate collection system.
c. The insulation material encasing the pipes has deteriorated and appeared to be fastened to the pipes with
silver duct tape. Mr. Lacsamana agreed that the original pipe insulation had deteriorated, and that some of
it was indeed held together with duct tape; however, this has no bearing on the integrity of the piping
system. He also stated that heat wrap had been added to ensure that the collection lines and tank
connections would not freeze.
d. During the inspection, it was not possible to ascertain the fill level of the five leachate storage tanks or to
confirm that the telemetry system was functioning properly. The control panels are locked and no exterior
gauges are present that could provide any information about the system at all. Mr. Lacsamana stated that
any interruption in the telemetry system, any leakage in the collection system, and all alerts about the fill
level of the tanks are sent automatically to his email account.
9. As requested in the previous inspection report, Mr. Burbach contacted Jaclynne Drummond, SWS Compliance
Hydrogeologist, to discuss the integrity and maintenance of the leachate collection system.
10. As discussed during this inspection, please provide a copy of the maintenance record for the leachate
collection system for the period 2012 through 2016.
