The inspection was conducted at the West Bound I-40 DOT Rest Area in Catawba County, North Carolina. 2. The following firm truck was inspected: Vehicle Identification Number (VIN) Tag Number Capacity (gallons) IFDXF7DX5HDB04397 YA137101 2800 The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 3. The tank appeared to be structurally sound and bolted to the frame. 4. Valves and access ports appeared to be secure. Caps were present on the access ports. 5. The truck was properly marked in accordance with the lettering requirements with the exception of the following: a. The company name was measured to be less than 3 inches in height; b. The town name and permit number were only located on one side of the vehicle; On 3/23/2017, photos were sent confirming that items a and b have been completed, however, it was later determined that Cleveland instead of Mooresville (which was on the vehicle) should have been on the vehicle as the town name. This was corrected for this vehicle and the vehicle that was last inspected on 9/26/2016 (VIN IFDXF7DX9GDA05497) with photos documenting the corrections submitted on 3/27/2017. 6. No leaks were observed.
