OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) Site has been closed due to overapplications to Fields 1 and 4. Mr. Zamora requested an inspection of the site as per Permit Condition 20 to see if the site could be re-opened. 2) The required signage is present as you enter the site. 3) Access road is well maintained. 4) Field markers present. I had to locate the position of several markers with the GPS unit. Mr. Zamora is placing a metal disk at the locations so that he will know where to place the markers, white pvc pipes, the next time they are taken down. 5) Site has been closed. No septage discharges or application noticed on the site. 6) There is a good stand of bermudagrass on all fields. Mr. Zamora has applied a herbicide to control some broadleaf weeds. Some weeds still present. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed today. 9) Setbacks confirmed when checking markers. 10) Records not checked today. Site has been closed to septage applications. 11) No violations noticed today. Site is re-open to septage application. You may start applying septage to the fields again. Please be sure to closely monitor the application amounts to each field on a monthly and yearly basis so as to not exceed the monthly application rates listed in your nutrient management plan and your annual maximum application rate that is listed in your permit.
