STATUS OF PAST NOTED VIOLATIONS:
From the inspection report dated 04/05/16, a violation of Rule .0838(b)(1) and Permit Condition 7 was noted for the
maximum annual application rate of 50,000 gal/ac/yr being exceeded for Fields 2/2a and 4 in 2015. A Notice of
Violation was issued on 04/06/16 for the violation. Also, the maximum annual application amount was reduced by
47,900 gallons to Field 2/2a and by 142,250 gallons to Field 4 for 2016. The reduction in gallons is listed in Permit
Condition 7 of your permit, SLAS-67-05, issued on 04/11/16.

OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) The required signage was present as you get to the site.
2) The access road was in good condition.
3) No eastern corners markers observed for Field 4. Check the permit map that was sent with the recent permit.
4) No septage discharges noticed outside of the permitted field’s boundaries.
5) Some evidence of spot dumps on Field 1 near the tanks. See picture below. Please rake any areas were spot
dumps occur. Spot dumps will typically kill the crop in the area that they occur unless they are raked to prevent
matting of the crop.
6) The bermudagrass needs improvement in all of the active fields. See a picture of Field 4 below. The fields have
significant pressure from both broadleaf and grass weeds. You need to seek technical assistance from the
NCDA&CS Regional Agronomist or County Cooperative Extension Agent. Contact me for their information.
Failure to improve the bermudagrass stand will ultimately result in closure of the fields. Please document any
actions you do to improve the bermudagrass stand.
7) No septage application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) Setbacks not checked.
10) Reviewed records issues with Linda. Setup new computer logs to help track monthly and yearly septage
applications to the fields. Did not check the inactive fields.

Evidence of spot dump in Field 1. Bermudagrass coverage needs improvement on all
active fields. Picture taken of Field 4.
