STATUS OF PERMIT:
Active, Permit (PTC – phase VI & PTO – phases VI and VII) for 33G - LCID was issued September 16, 2014 and
expires November 04, 2020.

PURPOSE OF SITE VISIT:
Partial Facility Compliance Inspection

STATUS OF PAST NOTED VIOLATIONS:
None

OBSERVED VIOLATIONS:
None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1) The Operating hours are Monday - Friday from 7:30 AM to 6:00 PM.
2) Waste acceptance is to be only by city staff and city authorized contractors. The public is not allowed to use this
facility.
3) Access road is made of All-Weather construction. Main entrance to site has locking gate.
4) The service area is Nash & Edgecombe Counties.
5) Incoming load records are being put in the box at the on ramp of the LCID.
6) Also has a locked cable at the ramp of the LCID.
7) A rain gauge is being maintained by the WWTP and provides the rainfall data.
8) Cover appears to be generally adequate.
9) No erosion observed today on side slopes.
10) Records were not reviewed today.
11) Only LCID material was found on-site today.
12) Site now has permanent cell 7 markers for boundaries surrounding the cell. Will need to move a small amount of the
West side slope back inside of boundary. Will need to stabilize slope with mulch and grass after moving this area.
13) Overall Site looked good during this inspection.
