OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) Required signage present before you get to the site. 2) Access road was in good shape. 3) Need to check how the borders are marked for the pine tree fields. 4) Based on the permit map, discharges appear to be within the permitted boundaries of the site. 5) No application issues noticed today. Septage only being applied to the pine trees within Fields 1 and 2. Fields 3, 4, and 5 are inactive. 6) The pines appear to be ok. The stand is thinning out some with pipe trees dying. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed today. 9) Setbacks not checked. 10) Records not checked. 11) I collected a composite soil sample from both Field 1 and 2.
