Jacksonville, NC 28540 GPS COORDINATES: N: 34.83439 E: -77.52657 FACILITY CONTACT INFORMATION: Name: Mr. Tom Balaker Address: Same as above Telephone: 910-324-1420 Email address: PARTICIPANTS: Chester Cobb STATUS OF PERMIT: Active EXPIRES: 02/11/21 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: An observed violation to Rule .0841 (j) was noted on 05/27/16 for septage spills on the ground by the tanks and on one of the tanks. OBSERVED VIOLATIONS: 1) Septage appears to have leaked or flowed out of two of the 6,500 gallons tanks. See pictures below. Rule .0841(e)(1) states “Each detention and treatment facility shall be designed, constructed, and maintained in such a manner as to: Prevent leaks or the flow of septage out of the facility into the seasonally high water table, onto the ground surface, or into any surface waters.” The leaks or flows of septage out of the two 6,500 gallons tanks is a violation of Rule .0841(e)(1). All leaks need to be patched and the top sight glass holes in the two tanks need to be plugged or replaced. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 AREAS OF CONCERN AND COMMENTS: 1) “No Trespassing” sign present as you enter the site. 2) For the most part, the tanks were accessible for inspection. Be sure to keep a 5 ft area around the tanks free of vegetation and debris so that the tanks can be inspected. 3) Most of the tanks appeared to be structurally sound except for the middle 6,500 gallons steel tank. The three – 6,500 gallons tanks are grouped together. The side of the center 6,500 gallons tank facing towards the office has multiple holes (see picture below). The holes need to be patched. 4) There are holes to the top of the first two 6,500 gallons tanks that previously had glass sight glasses. See picture below. There is currently nothing in those holes. Septage has flowed out of both holes. New sight glasses need to be placed in the holes or the holes need to be plugged immediately. 5) No objectionable odors or vector issues noticed. 6) The three – 6,500 gallons steel tanks appear to be used the most. The 31,500 gallons tank did not appeared to have been used recently. Multiple leaks noticed from the center 6,500 gallons tank along the side facing the office. The top sight glass is missing from the center 6,500 gallons tank. The top sight glass is missing from the first 6,500 gallons tank.
