OBSERVED VIOLATIONS:
1) Septage appears to have leaked or flowed out of two of the 6,500 gallons tanks. See pictures below. Rule

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2) For the most part, the tanks were accessible for inspection. Be sure to keep a 5 ft area around the
tanks free of vegetation and debris so that the tanks can be inspected.

The three – 6,500 gallons tanks are grouped together. The side of the center 6,500 gallons tank
facing towards the office has multiple holes (see picture below). The holes need to be patched.
4) There are holes to the top of the first two 6,500 gallons tanks that previously had glass sight glasses.
See picture below. There is currently nothing in those holes. Septage has flowed out of both holes.
New sight glasses need to be placed in the holes or the holes need to be plugged immediately.
5) No objectionable odors or vector issues noticed.
6) The three – 6,500 gallons steel tanks appear to be used the most. The 31,500 gallons tank did not
appeared to have been used recently.
