STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 07/15/16.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Required signage present as you enter site.
2) Access road is well maintained.
3) Boundary markers present for all fields.
4) No septage discharges noticed outside of the permitted boundaries of the fields.
5) No application issues noticed today. No spot dumps or thick applications noticed.
6) It appears that the annual ryegrass or a small grain had been planted for the winter crop. A winter cover
crop is needed for applications in the winter.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) Setbacks not checked.
10) Records submitted with application for permit renewal. Application received on 02/10/16.
11) I collected a composite soil sample from each field. Soil samples taken from fields 1, 2, 3, and 4.
Appears that the fields may have been planted in
ryegrass or a small grain.
