Attached is correspondence provided to you last March for which concurrence was subsequently received by you per
the email below. As discussed on Tuesday, CCR‐11D is a well that was drilled last year (shown on figure in
attachment). Grout impacts were suspected due to high pH in the well, and plans were made to redrill the well. The
replacement well was inadvertently drilled about 300 feet away adjacent to an existing non‐CCR monitoring well
cluster. Having determined this to be the case, Duke would like to correctly redrill the replacement CCR‐11D well
adjacent to the CCR‐11 cluster and to abandon the initial, erroneously located CCR‐11D replacement well to prevent
confusion in the field. This email serves to inform you and to request concurrence that Duke may redrill the well and
abandon the erroneously located replacement well.

Drilling Request
Dear Ms. Werner,
Duke Energy Carolinas, LLC (DEC), Allen Steam Station requests concurrence from North Carolina
Department of Environmental Quality (NCDEQ) Division of Waste Management (Division) for drilling in
vicinity to the Allen Steam Station Retired Ash Basin (RAB) Ash Landfill (Permit No. 3612-INDUS-2008).
This drilling request is in effort to satisfy USEPA Hazardous and Solid Waste Management System; Disposal
of Coal Combustion Residuals From Electric Utilities (USEPA CCR) 40 CFR Part 257.91. Currently,
fourteen (14) groundwater monitoring wells are intended to be installed in the approximate locations shown
on the attached drawing titled "Proposed CCR Monitoring Well Locations". Upon completion of monitoring
well installation, Duke Energy will submit a well installation completion package to the Division noting
actual well locations and depths including drilling logs.
In addition, approximately six (6) existing wells are likely to be utilized as part of this landfill CCR
monitoring network. The existing wells which are planned to be incorporated into the network are also
indicated with text boxes on the attached figure. Installation of additional background and downgradient
monitoring wells is also planned around the station’s ash basin system to be included in the site-wide, multi-
unit groundwater monitoring network for the CCR Rule.
