Attached is a design modification to Sampson County Disposal, LLC's (Permit No. 82-02) Phase 3 Permit to
Construct. This design modification entails the installation of a 5-foot square gravel trench to intercept
groundwater flows observed in the west slope of Cell 10.

A revised installation report will be submitted that will document the current levels of the GGI.

Description of Modification(s):
Installation of a 5-foot square gravel trench with a 12-inch perforated pipe to intercept groundwater flows observed in the west slope of Cell 10. This trench was installed approximately 25-
feet west of the existing, permitted, GGI leg, upslope from the toe-of-slope. Three downpipes will drain this upper trench to the existing GGI leg.
Reason for Modification(s):
A groundwter seep in the west slope of Cell 10 is contributing to elevated water table measurements and must be controlled to meet regulatory requirements. These seeps are influencing
the water surface in the area of PZ-10A, 10B, and 10C. Seeps are flowing through a coarse sandy lense.
