This form was created to facilitate the preparation of a receptor survey for a given brownfield property. This is a guidance document
only, and Prospective Developers may complete and submit their own receptor survey, if they desire. Any receptor survey submitted
must be completed by a qualified environmental professional.
Complete "Property Characteristics" Section and all portions of other sections where an "X" has been placed in the far left
column under “Applicable to Site,” as determined by the DENR Brownfields Project Manager on a site specific basis.
The search-radius distances to potential receptors from the Property boundary will be determined by the DENR Brownfields
Project Manager on a site by site basis.
Search-radius distances are measured from the site Property Boundary unless otherwise indicated by the DENR
Brownfield’s Project Manager.

2 - A figure with the current zoning information is provided in Appendix B. The BF property is outlined in red. Zoning
information for Halifax County was obtained from Chris Rountree of the Halifax County planning department. According to
Mr. Rountree there is no zoning map of the county available. Edgecombe County and Nash County zoning were obtained
from the County GIS mapping web sites. A copy of the Edgecombe County Development Plan (zoning map) is included in
Appendix B. No changes to zoning or land use are planned.
1 - A site map is provided in Appendix A. Redevelopment of the site has included the removal of some small structures and
the refurbishment of the larger structures. No significant changes to the site have been made or are pending.

not obtain complete utility location maps for the site and immediately adjacent properties; however, telephone calls and
email correspondence, as well as the site reconnaissance provided utility location information. A map obtained from Century
Link indicating the fiber and cable lines along US 301 also provided in Appendix C. Photographs of utilities at the site and
surrounding properties are provided in Appendix D.
Municipal water is not available to
the site. A waterline in Edgecombe
County lines terminates
approximately 1,200 feet south
from the property boundary. A

Upstream
The existing on-site water supply
well, WSW-1, is 230 feet deep with
the pump set at 180 feet below
ground surface. Additional water
supply wells are either upgradient
of the site or across Fishing Creek, a
hydraulic barrier.No irrigation wells were
specifically identified within 1/2
mile of the Property boundary;
however, the wells noted at the
Franklin Center at Bricks may be
irrigation wells since they are
connected to municipal water.
A monitoring well network is on
site. See figure in Appendix C for
locations of monitoring wells.

Three transient community wells
are located across Fishing Creek
from the site. These wells serve the
Franklin Center at Bricks, a
conference center. According to the
Edgecombe Co. Water Department,
the center is also connected to
municipal water.
4 - On-site water supply wells WS-1 and WS-2 were sampled on 5/2/2007. The laboratory results indicated no constituents
of concern at concentrations above the 15A NCAC 2L Groundwater Standards. Water supply well WS-2 was abandoned on
7/22/2016. A copy of the abandonment record is provided in Appendix E. The locations of these two wells are on the Septic
Fields Map in Appendix C.

Fishing Creek is fed by streams further inland. Fishing Creek is a tributary of
the Tar River.
