The subject site is an approximate 6.24 acre parcel. Any exceptions to or deletions from this practice are described in Section 2.3 of this report. The site is located at 157 Industrial Drive in King, North Carolina. Based on a review of historical information, it appears the site was developed with a commercial building in 1967. The building at the site was periodically renovated and expanded. The site is developed with an approximate 13,440 square foot industrial building and asphalt paved areas. The building is currently unoccupied. The building contains office space, restrooms, manufacturing areas, storage areas, shipping and receiving and waiting areas. The building was formerly used to manufacture medical electronic devices. Light staining was observed on the concrete floor slab in several areas within the building. The site currently receives water and sanitary sewer services from the City of King. The building is heated with electric heat pumps. Floor drains were observed beneath emergency wash stations and janitorial closets in the building. A sump pump was observed in the northwestern portion of the building. Several monitoring wells were observed north, east and west of the building. Water supply well caps were observed southeast of the building. The well house caps were locked. A former waste treatment system and drain field are located north of the building. We did not observe stressed vegetation, stained soils or evidence of underground storage tanks (USTs) at the site. The Standard Environmental Records (SER) search provided by Environmental Data Resources (EDR) identified Carolina Medical Electronics on the Incident Management Database (IMD). We have reviewed several assessment reports provided by Mr. Joe Landry with DM Squared, the prospective purchaser of the site. According to information reviewed, a separate septic system was installed at the site in 1977 to handle solvent washings from the fabrication of printed circuit boards. The discharge system consisted of a stone drain sink which was connected to a 100-gallon holding tank. The liquid discharged into the sink was maintained in the holding tank until pH adjustments were made and then discharged into a 200-gallon holding tank and discharged onto the northern portion of the site through leach lines. ECS Project No. 09-21034 May 01, 2012 - 2 - According to a Monitoring Well Installation Report, prepared by Trigon Engineering Consultants, Inc. dated February 21, 1996, two monitoring wells (MW-1 and MW-2) were installed in the vicinity of the discharge system. Soil samples were collected at five-foot intervals and screened in the field. Soil sample screening did not detect volatile organic vapors in the soil samples screened. Soil samples were not submitted for laboratory analysis. Groundwater samples were collected from each monitoring well and analyzed for volatile organic compounds (VOCs), semi-volatile organic compounds (SVOCs), RCRA Metals and gold. Laboratory analysis of the groundwater samples detected trichloroethene, tetrachloroethene (perchloroethene) and lead in excess of the NCDENR 2L Groundwater Quality Standards. Based on the laboratory results, the NCDENR Division of Water Quality (DWQ) issued a Notice of Regulatory Requirements (NORR) requiring that a Comprehensive Site Assessment (CSA) be performed at the site. According to a Comprehensive Site Assessment Report performed by Mineral Springs Environmental, P.C. (Mineral Springs), dated February 22, 2010, the two holding tanks were removed on October 21, 1996 and soils excavated from beneath the two tanks were land applied on other portions of the site. In addition, two aggressive fluids/vapor recovery events (AFVR) were conducted at the site and one additional monitoring well (MW-3) was installed at the site prior to 1998. According to the CSA report, a receptor survey was performed in 2009 to locate water supply wells within 1,500 feet of the site. A total of five water supply wells were located during the survey. However, only two wells were identified to be within 1,000 feet of the site and these wells were determined to not be in use. According to the report, six Type II monitoring wells (MW-4 through MW-8) were installed to help delineate the horizontal extent of the contaminant plume at the site including one upgradient well (MW-9). In addition to the shallow monitoring wells, one Type III deep aquifer monitoring well (MW-1D) was installed in the vicinity of MW-1. Laboratory analysis of groundwater samples collected from the monitoring wells detected several compounds in excess of the NC2L Standards. Tetrachloroethene (PCE) was the primary compound of concern. A PCE isoconcentration map indicates the contaminant plume extends across the majority of the site. The CSA is included in Appendix IV. ECS Project No. 09-21034 May 01, 2012 - 3 - According to a Comprehensive Site Assessment Addendum Report dated September 13, 2010, the NCDENR issued a correspondence requesting additional soil and groundwater assessment at the site. A meeting was conducted at the site in June 2010 and the NCDENR requested that the two water supply wells located south and west of the site be abandoned, installation of two additional monitoring wells south of the building, and collection of four soil samples in the vicinity of the former holding tank. The water supply wells were abandoned in June 2010. In July, 2010, two Type II monitoring wells (MW-10 and MW-11) were installed south of the building to further delineate the contaminant plume. Laboratory analysis of the groundwater samples collected from MW-10 and MW-11 did not detect the targeted compounds in excess of laboratory quantitation limits. Laboratory analysis of soil samples collected detected tetrachloroethene at a concentration below the NCDENR Industrial/Commercial Maximum Soil Contaminant Concentration (MSCC). The CSA and CSA addendum report recommend the completion of a Corrective Action Plan (CAP) to address groundwater impact at the site. The CAP has not been completed at the time of this study. The contaminant plume impacting groundwater at the site is considered to be a recognized environmental condition of the site. The SER search identified Parker Chevrolet, located approximately 2,150 feet southeast of the site as having a documented release of petroleum products. However, based on the distance and/or the groundwater flow direction inferred from the surface topography, the release from this property is not expected to impact the site. As documented and qualified by this report, this assessment has revealed the following recognized environmental conditions in connection with the property. On-Site A documented release of chlorinated solvents has been identified at the site. The contaminant plume has been vertically and horizontally delineated. The NCDENR has requested a Corrective Action Plan that has not been completed at this time. The contaminant plume impacting groundwater at the site is considered to be a recognized environmental condition of the site. It is likely that the incident at the site will be managed by the Inactive Hazardous SitesBranch (IHSB) section of the NCDENR in the future. The IHSB may require additional investigation. It is also possible that the solvent rinse could be considered hazardous waste. If so, the holding tanks could be considered Solid Waste Management Unit (SWMS) and could be regulated by RCRA depending on when the discharge began and ended. Additional considerations Based on the age of the building and the types of materials observed, asbestos containing building materials may be presentin the building. Prior to planned renovations and/or demolition of the building, ECS recommends an asbestos survey be performed on the building at the site. Off-Site ECS Project No. 09-21034 May 01, 2012 - 4 - No off-site recognized environmental conditions were identified by this study. This Executive Summary is an integral part of the Phase I Environmental Site Assessment report. ECS recommends that the report be read in its entirety. ECS Project No. 09-21034 May 01, 2012 - 5 - 2.0 INTRODUCTION 2.1 Purpose and Reason for Performing Phase I ESA The purpose of the ESA was to: · evaluate the probability of impact to the surface water, groundwater and/or soils within the property boundaries through a review of regulatory information and a reconnaissance of the subject site and vicinity; · evaluate historical land usage to identify previous conditions that could potentially impact the environmental condition of the site; · evaluate the potential for on-site and off-site contamination; · provide a professional opinion regarding the potential for environmental impact at the site, and a list of Recognized Environmental Conditions; and, · conduct all appropriate inquiry as defined by ASTM-E 1527-05. The reason for the ESA is to determine the possibility of contamination that has not yet been identified, and to submit with an application for entry into the North Carolina Brownfields Program. 2.2 Scope of Services The environmental assessment was conducted in accordance with ASTM E 1527-05 and EPA Standards and Practices for All Appropriate Inquiry (40 CFR §312.10). The environmental assessment was conducted under the supervision or responsible charge of an individual that qualifies as an environmental professional, as defined in 40 CFR §312.10. 2.3 Limitations The ESA involved a reconnaissance of the site and contiguous properties and a review of regulatory and historical information in accordance with the ASTM standard and EPA regulation referenced herein. No non-scope considerations or additional issues such as asbestos, radon, wetlands or mold were investigated, unless otherwise described in Section 7.0 of this report. ECS Project No. 09-21034 May 01, 2012 - 6 - The conclusions and/or recommendations presented within this report are based upon a level of investigation consistent with the standard of care and skill exercised by members of the same profession currently practicing in the same locality under similar conditions. The intent of this assessment is to identify the potential for recognized environmental conditions in connection with the site; however, no environmental site assessment can completely eliminate uncertainty regarding the potential for recognized environmental conditions in connection with the site. The findings of this ESA are not intended to serve as an audit for health and safety compliance issues pertaining to improvements or activities at the site. ECS is not liable for the discovery or elimination of hazards that may potentially cause damage, accidents or injury. Observations, conclusions and/or recommendations pertaining to environmental conditions at the subject site are necessarily limited to conditions observed, and or materials reviewed at the time this study was undertaken. It was not the purpose of this study to determine the actual presence, degree or extent of contamination, if any, at this site. This could require additional exploratory work, including sampling and laboratory analysis. No warranty, expressed or implied, is made with regard to the conclusions and/or recommendations presented within this report. ASTM E-1527-05 defines a "recognized environmental condition" as "the presence or likely presence of any hazardous substances or petroleum products on a property under conditions that indicate an existing release, a past release, or a material threat of a release of any hazardous substances or petroleum products into structures on the property or into the ground, groundwater, or surface water of the property. The term includes hazardous substances or petroleum products even under conditions in compliance with laws. The term is not intended to include de minimis conditions that generally do not present a threat to human health or the environment and that generally would not be the subject of an enforcement action if brought to the attention of appropriate governmental agencies." ASTM E-1527-05 defines a “business environmental risk” as “a risk which can have a material environmental or environmentally-driven impact on the business associated with the current or planned use of a parcel of commercial real estate”. Client-imposed limitations and site condition limitations, if encountered, are detailed in Section 6.1 Methodology and Limiting Conditions. This report is provided for the exclusive use of DM Squared. This report is not intended to be used or relied upon in connection with other projects or by other unidentified third parties. The use of this report by any undesignated third party or parties will be at such party’s sole risk and ECS disclaims liability for such third party use or reliance. ECS Project No. 09-21034 May 01, 2012 - 7 - 3.0 SITE DESCRIPTION 3.1 Site Location and Legal Description The site referred to as “subject property,” “site” or “the property” is an approximate 6.24 acre tract located at 157 Industrial Drive in King, North Carolina (Figure 1, Appendix 1). The site is developed with an approximate 13,440 square foot industrial facility. The property is further identified by Stokes County as parcel number 5991-07-58-2808. According to the Stokes County GIS website, the building was constructed in 1967. 3.2 Physical Setting and Hydrogeology As determined from the USGS Topographic Maps, King, Pinnacle, Vienna and Rural Hall, North Carolina Quadrangles (Figure 1), an unnamed tributary to Crooked Run Creek is located southwest of the site. Elevations at the site range from approximately 1,080 feet above mean sea level along the eastern, southern and western property boundaries to approximately 1,060 feet above mean sea level along the north property boundary. Surface runoff on the site is generally to the west-northwest. Limited areas to the south and southeast appear to be upgradient of the site based on the groundwater flow direction inferred from the surface topography and confirmed by water levels measured in monitoring wells located on the site. According to the Geologic Map of North Carolina (1985), the subject property is situated in the Milton Belt of the Piedmont Physiographic Province with the underlying bedrock composed of metamorphosed mafic rock. The Piedmont Physiographic Province is characterized by gently rolling topography, deeply weathered bedrock, and a relative paucity of solid outcrop. Rocks are strongly weathered in the Piedmont's humid climate and bedrock is generally buried under a thick (2-20 m) blanket of saprolite. Outcrops are commonly restricted to stream valleys where saprolite has been removed by erosion. The topography becomes somewhat more rugged with proximity to the Blue Ridge, where local monadnocks of more resistant rock occur. The soils encountered in this area are the residual product of in-place chemical weathering of rock presently underlying the site. In general, shallow unconfined groundwater movement within the overlying soils is controlled largely by topographic gradients. However, as the groundwater percolates downward to the bedrock, it becomes controlled by the orientation of the rock fracture systems. Thus, the direction of groundwater movement may not be consistent with the reflecting topography. Recharge occurs primarily by infiltration along higher elevations and typically discharges into streams or other surface water bodies. The elevation of the shallow water table is transient and can vary with seasonal fluctuations in precipitation. Movement of groundwater under water table conditions is generally from higher to lower elevations. As such, shallow groundwater would be expected to flow to the north. Actual site groundwater flow direction has been determined from groundwater information determined from the installation of groundwater monitoring wells at the site. ECS Project No. 09-21034 May 01, 2012 - 8 - 3.3 Current Use and Description of the Site The site is an approximate 6.24 acre tract that is currently developed with an approximate 13,440 square foot industrial building. The building is currently unoccupied. 3.4 Current Uses of the Adjoining Properties The site is bound on the north by a gravel driveway and Chamberlain Industries; on the east by Industrial Drive Extension; on the south by Kates Farm Road; and, on the west by agricultural buildings and undeveloped land. Additional information regarding adjacent properties is provided in Section 6.3. ECS Project No. 09-21034 May 01, 2012 - 9 - 4.0 USER PROVIDED INFORMATION Mr. Joseph H. Landry, the prospective developer, has completed the ASTM E 1527-05 User Questionnaire. Mr. Landry is the user of this Phase I ESA. Section 4.0 is based on the completed questionnaire. 4.1 Title Information At the time of this report, ECS was not provided with the chain-of-title information for the subject site. The lack of this information is considered to be a data gap. However, based on the historical and regulatory records information presented, ECS considers this data gap to be non-significant. This data gap has not affected our ability to identify RECs. If ECS is provided with this information at a later date an addendum will be issued to this report if the information indicates a material difference in our findings. 4.2 Environmental Liens or Activity and Use Limitations Mr. Landry indicated that he can provide data on this issue. Mr. Landry provided ECS with several environmental reports performed by Trigon Engineering and Mineral Springs Environmental, p.c. Based on this information, environmental liens or activity and use limitations have not been recorded at the site. 4.3 Specialized Knowledge Mr. Landry indicated that he does not have specialized user knowledge or experience related to the property or nearby properties. 4.4 Commonly Known or Reasonably Ascertainable Information Mr. Landry indicated that he possesses commonly known or reasonably ascertainable information about the property. This information includes the past uses of the property, specific chemicals that are or were once present at the property, spills or other chemical releases that have taken place at the property, and environmental cleanups that have taken place at the property. 4.5 Valuation Reduction for Environmental Issues Mr. Landry indicated that the purchase price being paid for this property reflects the fair market value of the property. 4.6 Owner, Property Manager, and Occupant Information Mr. Landry indicated that the site is owned by King Investment Partners. 4.7 Degree of Obviousness Mr. Landry indicated that based on his knowledge and experience related to the property, there are indicators that point to the presence of contamination at the property. ECS Project No. 09-21034 May 01, 2012 - 10 - 5.0 RECORDS REVIEW A regulatory records search of ASTM standard and supplemental databases was conducted for the site and is included in Appendix III. The regulatory search report in the appendix includes additional details about the regulatory databases that were reviewed. The regulatory records search involves searching a series of databases for facilities that are located within a specified distance from the subject property. The ASTM standard specifies an approximate minimum search distance from the subject site for each database. Pursuant to ASTM, the approximate minimum search distance may be reduced for each standard environmental record except for Federal NPL site list, and Federal RCRA TSD list. According to ASTM, government information obtained from nongovernmental sources may be considered current if the source updates the information at least every 90 days or, for information that is updated less frequently than quarterly by the government agency, within 90 days of date the government agency makes the information available to the public. The following table indicates the standard environmental record sources and the approximate minimum search distances for each. While this attempt at verification is made with due diligence, ECS cannot guarantee the accuracy of the record(s) search beyond that of information provided by the regulatory report(s). Mapped and unmapped sites identified in the regulatory report(s) that are not addressed below were field verified and are not believed to be within the approximate minimum search distance and are excluded from this ESA report. ECS makes no warranty regarding the accuracy of the database report information included within the regulatory report(s). The Standard Environmental Records (SER) search was provided by EDR. Search distances recommended by ASTM 1507-E were maintained for the databases reviewed. 5.1 Federal Databases Federal National Priorities List (NPL) The NPL is a subset of Comprehensive Environmental Response, Compensation, and Liability Information System (CERCLIS) and identifies “superfund” sites that have had documented contamination incidents. The site was not identified on the NPL. No off-site properties within the minimum search distance of inquiry were reported as being on the NPL. Federal Delisted NPL The Delisted NPL identifies sites previously listed on the NPL where no further response is appropriate. The site was not identified on the Delisted NPL. No off-site properties within the minimum search distance of inquiry were reported as being on the Delisted NPL. Federal CERCLIS CERCLIS contains data on potential hazardous waste sites that have been reported to the United States Environmental Protection Agency (USEPA). CERCLIS contains sites that are either proposed to or on the NPL and sites which are in the screening and assessment phase for possible inclusion on the NPL. The site was not identified on the CERCLIS database. No off-site properties within the minimum search distance of inquiry were reported as being on the CERCLIS database. Federal CERCLIS No Further Remedial Action Planned (NFRAP) CERCLIS sites designated as NFRAP have been removed from CERCLIS. NFRAP sites may be sites where, following an initial investigation, no contamination was found, contamination was removed quickly without the need for the site to be placed on the NPL, or the contamination was not serious enough to require federal Superfund action or NPL consideration. ECS Project No. 09-21034 May 01, 2012 - 12 - The site was not identified on the CERCLIS NFRAP. No off-site properties within the minimum search distance of inquiry were reported as being on the CERCLIS NFRAP. Federal Corrective Action Report (CORRACTS) CORRACTS identifies hazardous waste handlers that have been subject to corrective action under Resource Conservation and Recovery Act (RCRA). The site was not identified by CORRACTS. No off-site properties within the minimum search distance of inquiry were reported as being on CORRACTS. Federal Resource Conservation and Recovery Information System (RCRIS) – Treatment, Storage and Disposal (TSD) Facilities RCRIS identifies facilities that treat, store or dispose of hazardous wastes as defined by the RCRA. TSDs treat, store or dispose of hazardous waste. The site was not identified as a hazardous waste TSD facility. No off-site properties within the minimum search distance of inquiry were reported as being a hazardous waste TSD facility. Federal RCRIS - Generators RCRIS identifies facilities that generate hazardous wastes as defined by the RCRA. Conditionally exempt small quantity generators generate less than 100 kilograms of hazardous waste, or less than 1 kilogram of acutely hazardous waste, per month. Small quantity generators (SQGs) generate between 100 and 1,000 kilograms of hazardous waste per month. Large quantity generators (LQGs) generate more than 1,000 kilograms of hazardous waste or more than 1 kilogram of acutely hazardous waste per month. The site was not identified as a hazardous waste generator. No off-site properties within the minimum search distance of inquiry were reported as being a hazardous waste generator. Federal Engineering Controls (EC) List The Federal EC list identifies engineering controls including various forms of caps, building foundations, liners, and treatment methods used to eliminate pathways for regulated substances to enter environmental media or affect human health. The site was not identified on the EC list. Federal Institutional Controls (IC) List The Federal IC list identifies institutional controls including administrative measures, such as groundwater use restrictions, construction restrictions, property use restrictions, and post remediation care requirements intended to prevent exposure to contaminants. ECS Project No. 09-21034 May 01, 2012 - 13 - The site was not identified on the IC list. Federal Emergency Response Notification System (ERNS) The ERNS list is a national database that stores and records information on reported releases of hazardous substances, including petroleum products. The site was not identified on the ERNS list. 5.2 State Databases State Brownfield Projects Inventory The State Brownfield Projects Inventory identifies brownfield projects inventoried with the North Carolina Department of Environment and Natural Resources (NCDENR). The properties in the inventory are working toward a brownfield agreement for cleanup and liability control. The site was not identified on the State Brownfield Projects Inventory. No off-site properties within the minimum search distance of inquiry were reported as being on the State Brownfield Projects Inventory. Old Landfill Inventory (OLI) The OLI is maintained by the NCDENR. The inventory does not include no further action sites and other agency lead sites. The site was not identified on the OLI. NC Unmapped (Orphan) Facilities and Sites Several properties were identified on the Orphan Summary List prepared by EDR. These facilities are considered by EDR as unmappable because the facility information in the database is insufficient and does not report accurate facility location information. ECS Project No. 09-21034 May 01, 2012 - 14 - Leaking Underground Storage Tank (LUST) List The LUST list, maintained by NCDENR-Department of Waste Management (DWM), is a record of reported leaking underground storage tank incidents. The LUST list may also identify properties that have had soil and/or groundwater contamination associated with documented releases from aboveground storage tanks, surface spills and other sources. The site was not identified on the LUST list. One or more off-site properties within the minimum search distance of inquiry were reported as being on the LUST database. · Parker Chevrolet (Former), 329 Newsome Road - located approximately 2,150 feet southwest of the site. Based on the distance, the release at this property is not expected to impact the site. Registered Underground Storage Tank (UST) List The Registered UST List inventories underground storage tanks registered with the NCDENR, Division of Water Quality. This list does not identify USTs that have not been registered with the NCDENR, such as home heating oil tanks and other unregulated tanks. The site was not identified on the Registered UST list. No off-site properties within the minimum search distance of inquiry were reported as being on the Registered UST list. State Responsible Party Voluntary Action (VCP) Sites The VCP Sites is a listing of sites that parties wish to remediate voluntarily. The program is administered by the NCDENR. The site was not identified on the VCP Sites list. No off-site properties within the minimum search distance of inquiry were reported as being on the VCP Sites list. State Hazardous Waste Sites (SHWS) Inventory State hazardous waste site records are the states’ equivalent to CERCLIS. These sites may or may not be already listed on the federal CERCLIS database. The site was not identified on the SHWS Inventory. No off-site properties within the minimum search distance of inquiry were reported as being on the SHWS Inventory. Incident Management Database (IMD) The IMD, maintained by NCDENR-DWM, is a record of groundwater and soil contamination incidents. The site was identified on the IMD. ECS Project No. 09-21034 May 01, 2012 - 15 - One or more off-site properties within the minimum search distance of inquiry were reported as being on the IMD. · Carolina Medical, Inc., 157 Industrial Drive - site. We have reviewed several assessment reports provided by Mr. Joe Landry with DM Squared, the prospective purchaser of the site. According to information reviewed, a separate septic system was installed at the site in 1977 to handle solvent washings from the fabrication of printed circuit boards. The discharge system consisted of a stone drain sink which was connected to a 100-gallon holding tank. The liquid discharged into the sink was maintained in the holding tank until pH adjustments were made and then discharged into a 200-gallon holding tank and discharged onto the northern portion of the site through leach lines. According to a Monitoring Well Installation Report, prepared by Trigon Engineering Consultants, Inc. dated February 21, 1996, two monitoring wells (MW-1 and MW-2) were installed in the vicinity of the discharge system. Soil samples were collected at five-foot intervals and screened in the field. Soil sample screening did not detect volatile organic vapors in the soil samples screened. Soil samples were not submitted for laboratory analysis. Groundwater samples were collected from each monitoring well and analyzed for volatile organic compounds (VOCs), semi-volatile organic compounds (SVOCs), RCRA Metals and gold. Laboratory analysis of the groundwater samples detected trichloroethene, tetrachloroethene (perchloroethene) and lead in excess of the NCDENR 2L Groundwater Quality Standards. Based on the laboratory results, the NCDENR Division of Water Quality (DWQ) issued a Notice of Regulatory Requirements (NORR) requiring that a Comprehensive Site Assessment (CSA) be performed at the site. According to a Comprehensive Site Assessment Report performed by Mineral Springs Environmental, P.C. (Mineral Springs), dated February 22, 2010, the two holding tanks were removed on October 21, 1996 and soils excavated from beneath the two tanks were land applied on other portions of the site. In addition, two aggressive fluids/vapor recovery events (AFVR) were conducted at the site and one additional monitoring well (MW-3) was installed at the site prior to 1998. According to the CSA report, a receptor survey was performed in 2009 to locate water supply wells within 1,500 feet of the site. A total of five water supply wells were located during the survey. However, only two wells were identified to be within 1,000 feet of the site and these wells were determined to not be in use. According to the report, six Type II monitoring wells (MW-4 through MW-8) were installed to help delineate the horizontal extent of the contaminant plume at the site including one upgradient well (MW-9). In addition to the shallow monitoring wells, one Type III deep aquifer monitoring well (MW-1D) was installed in the vicinity of MW-1. Laboratory analysis of groundwater samples collected from the monitoring wells detected several compounds in excess of the NC2L Standards. Tetrachloroethene (PCE) was the primary compound of concern. A PCE isoconcentration map indicates the contaminant plume extends across the majority of the site. The CSA is included in Appendix IV. According to a Comprehensive Site Assessment Addendum Report dated ECS Project No. 09-21034 May 01, 2012 - 16 - September 13, 2010, the NCDENR issued a correspondence requesting additional soil and groundwater assessment at the site. A meeting was conducted at the site in June 2010 and the NCDENR requested that the two water supply wells located south and west of the site be abandoned, installation of two additional monitoring wells south of the building, and collection of four soil samples in the vicinity of the former holding tank. The water supply wells were abandoned in June 2010. In July, 2010, two Type II monitoring wells (MW-10 and MW-11) were installed south of the building to further delineate the contaminant plume. Laboratory analysis of the groundwater samples collected from MW-10 and MW-11 did not detect the targeted compounds in excess of laboratory quantitation limits. Laboratory analysis of soil samples collected detected tetrachloroethene at a concentration below the NCDENR Industrial/Commercial Maximum Soil Contaminant Concentration (MSCC). The CSA and CSA addendum report recommend the completion of a Corrective Action Plan (CAP) to address groundwater impact at the site. The CAP has not been completed at the time of this study. The contaminant plume impacting groundwater at the site is considered to be a recognized environmental condition of the site. · Parker Chevrolet (Former), 329 Newsome Road - located approximately 2,150 feet southwest of the site. Based on the distance, the release at this property is not expected to impact the site. Solid Waste Facilities/Historic Landfill List (HIST LF) State Historic Solid Waste Facility site records are maintained by the North Carolina Department of Environment and Natural Resource. The site was not identified on the HIST LF listing in the search report. No off-site properties within the minimum search distance of inquiry were reported as being on the HIST LF listing in the search report. Leaking Underground Storage Tanks (LUST TRUST) The LUST TRUST, maintained by NCDENR-DWM, contains information about claims against the State Trust Funds for reimbursements for expenses incurred while remediating Leaking USTs. The site was not identified on the Leaking Underground Storage Tank (LUST TRUST) list. No off-site properties within the minimum search distance of inquiry were reported as being on the LUST TRUST database. Aboveground Storage Tank (AST) Database The AST Database is a list maintained by the NCDENR facilities with ASTs that have a capacity greater than 21,000-gallons. The site was not identified on the AST Database. ECS Project No. 09-21034 May 01, 2012 - 17 - No off-site properties within the minimum search distance of inquiry were reported as being on the AST database. Solid Waste Facilities/Landfill(SWL) List The North Carolina Solid Waste Section maintains a list of permitted solid waste facilities. These facilities may be active or inactive facilities or open dumps that failed to meet RCRA Subtitle D Section 4004 criteria for solid waste landfills or disposal sites. The site was not identified on the North Carolina SWL. No off-site properties within the minimum search distance of inquiry were reported as being on the North Carolina SWL. 5.3 Historical Use Information Sanborn Fire Insurance Map Review Sanborn Fire Insurance Maps are not available for the site or the properties in the immediate vicinity of the site. The absence of such maps generally indicates that the site is located in an area where Sanborn Fire Insurance Maps were not produced because the area was rural or it was not economically feasible. ECS does not consider the lack of Sanborn Maps to be a data gap, given the significant amount of historical information obtained from our interviews, the USGS topographic map, aerial photographs and city directories. Aerial Photograph Review ECS reviewed aerial photographs of the site and immediately surrounding properties for evidence of former usage which may constitute a potential for environmental concern. The aerial photographs were obtained from the Stokes County Natural Resources Conservation Service (NRCS), the Stokes County Farm Services Agency, the Stokes County Planning Department and the Stokes County Geographic Information Systems (GIS) Department. The aerial photographs reviewed were dated 1941, 1955, 1967, 1974, 1985, 1995, 2003 and 2009 (Figure 2). On the 1941 and 1955 aerial photographs the site contains agricultural fields and undeveloped land. Agricultural barns, single family residences, agricultural fields and and undeveloped land are visible in the vicinity of the site. On the 1967 through 2003 aerial photographs, a commercial building consistent with the building observed during this study is visible in the center of the site. The building has been periodically expanded between 1967 and 2003. Highway 52 is visible to the north and east of the site on the 1967 aerial photograph and commercial buildings are visible south of the site beginning on the 1985 aerial photograph. Storage buildings and mobile homes consistent with Chamberlain Industries are visible north of the site beginning on the 1985 aerial photograph and buildings consistent with Christian Village Retirement Center are visible further northeast on the 2003 aerial photograph. ECS Project No. 09-21034 May 01, 2012 - 18 - The 2009 aerial photograph generally depicts the conditions encountered during this study. The site contains a commercial building and asphalt paved areas. The surrounding properties contain commercial businesses to the north and south, Highway 52 to the north and east, and single family residences and undeveloped land to the east and west. City Directory Review One of the ASTM standard historic sources to be reviewed for previous site use are local street directories, commonly known as City Directories. The purpose of the directory review is to identify past occupants of the site, adjoining properties, or nearby properties. In some rural areas, street directories information is limited. We have reviewed directories at the Stokes County Public Library to determine past occupants of the site and properties immediately surrounding the site. The directories reviewed were dated 1955, 1961, 1966, 1970, 1975, 1980, 1985, 1989, 1995, 2000, 2005 and 2010. The site address is 157 Industrial Drive. The directories dated 1970 through 1989 and 2000 and 2005 identify Carolina Medical Electronics at the site address. Residential occupants and sparse retail and commercial businesses were identified in the vicinity of the site. No gasoline stations, auto repair facilities or manufacturing facilities were identified at site addresses in the vicinity of the site in the directories reviewed. Other Standard Historical Sources In accordance with the ASTM Standard, other historical sources should be reviewed, if necessary and if the information is likely to be useful, to obtain historical site use information. Other Standard Historical Sources may include property tax files, recorded land title records, historic USGS topographic maps, building department records, and zoning or land use records. Based on the information obtained from aerial photographs, city directories and interviews, other historical sources were not reviewed. Other Local Historical Sources Other credible historical sources may be reviewed to identify past uses of the property. These sources may include internet sites, county or State road maps, historical society documents, or local library information. We contacted the King Fire Department to provide us with information pertaining to fire department responses, incidents, UST removals and/or chemical and hazardous spills at or in the vicinity of the site. According to Ms. Lisa Taylor, the fire department has no records of UST removals or chemical or hazardous spills at or in the vicinity of the site. Freedom of Information (FOIA) Requests NO FOIA requests were made during this study. ECS Project No. 09-21034 May 01, 2012 - 19 - 6.0 SITE AND AREA RECONNAISSANCE 6.1 Methodology and Limiting Conditions Mr. Toby Benfield of ECS conducted the field reconnaissance on March 26, 2012. The weather at the time of the reconnaissance was warm and clear. Observations were made from a walking reconnaissance around the perimeter boundary, along several transects through the property and in and around the building. Photographs, taken at the time of the site reconnaissance, are included in Appendix VI. 6.2 On-Site Features The site is located at 157 Industrial Drive in King, North Carolina. The site is developed with an approximate 13,440 square foot industrial building and asphalt paved areas. The site receives water and sanitary sewer services from the City of King. The building is heated with electric heat pumps. The building (Photograph 1) is currently unoccupied. The building contains office space, restrooms, manufacturing areas, storage areas, shipping and receiving and waiting areas. The building was formerly used to manufacture medical electronic devices. Light staining was observed on the concrete floor slab in several areas within the building. A sump pump was observed in the northwestern portion of the building. Floor drains were observed beneath emergency wash stations and janitorial closets in the building. Several monitoring wells were observed north, east and west of the building. Water supply well caps were observed southeast of the building (Photograph 2). The well house caps were locked. We did not observe stressed vegetation, stained soils or evidence of USTs at the site. Underground or aboveground storage tanks We did not observe aboveground storage tanks or evidence of underground storage tanks at the site. Strong, pungent or noxious odors We did not notice strong, pungent or noxious odors at the site. Surface waters We did not observe streams or other surface waters located on the site. Standing pools of liquid likely containing petroleum or hazardous substances We did not observe standing pools of liquid at the site. Drums or containers of petroleum or hazardous substances greater than five- gallons We did not observe drums or containers of petroleum or hazardous substances greater than five-gallons at the site. ECS Project No. 09-21034 May 01, 2012 - 20 - Drums or containers of petroleum or hazardous substances less than or equal to five-gallons We did not observe drums or containers of petroleum or hazardous substances less than or equal to five-gallons at the site. Unidentified opened or damaged containers of hazardous substances or petroleum products We did not observe unidentified opened or damaged containers of hazardous substances or petroleum products at the site. Known or suspect PCB-containing equipment (excluding light ballasts) The following observations were made with regard to known or suspect PCB-containing equipment on the site: Pole mounted transformers were observed at the site. Staining was not observed on the transformers, poles or ground surfaces in the vicinity of the transformers. Stains or corrosion to floors, walls or ceilings The following observations were made with regard to stains or corrosion to floors, walls or ceilings: Light staining was observed on the concrete floor slab in several areas within the building. The staining appears to be confined to the concrete slab and is not considered to be a recognized environmental condition. Floor drains and sump pumps The following observations were made with regard to floor drains or sump pumps at the site: A floor drain and sump pump were observed in the northwestern portion of the building. Pits, ponds or lagoons We did not observe pits, ponds or lagoons at the site. Stained soil or pavement We did not observe stained soils or pavement at the site. Stressed vegetation We did not observe stressed vegetation at the site. Solid waste mounds or non-natural fill materials We did not observe solid waste mounds or non-natural fill materials at the site. Wastewater discharges into drains, ditches or streams We did not observe wastewater discharges at the site. ECS Project No. 09-21034 May 01, 2012 - 21 - Groundwater wells including potable, monitoring, dry, irrigation, injection and/or abandoned The following observations were made with regard to groundwater wells at the site: Several monitoring wells were observed north, south, east, and west of the building. Water supply well caps were observed southeast of the building. The well house caps were locked. Municipal water from the City of King is utilized at the site. Septic systems or cesspools The following observations were made with regard to evidence of septic systems or cesspools at the site: The site has historically utilized a septic system. Elevators We did not observe elevators on the site. Dry Cleaning We did not observe dry-cleaning operations on the site. Onsite Emergency Electrical Generators We did not observe emergency generators on the site. 6.3 Adjoining and Nearby Properties Contiguous and nearby properties were observed during a walking and vehicular reconnaissance of the property boundary and public places. The site is located in an residential/commercial area of King. The site is bound on the north by a gravel driveway and Chamberlain Industries. Storage Buildings and Mobile Homes, single family residences and undeveloped wooded land are located further north. US Highway 52 is located north and east of the site. The site is bound on the east by a Industrial Drive. US Highway 52, Christian Village Retirement Community, single family residences and undeveloped land. Retail and commercial are located further east. The site is bound on the south by Kates Farm Road. Burning Bush Community Church, Nathan Outdoor Products, Merfin Industries, Indoor Urban Paintball, Buck Racing, single family residences and undeveloped land are located further south. The site is bound on the west by agricultural buildings and undeveloped land. Single family residences, agricultural fields and undeveloped land are located further west of the site. ECS Project No. 09-21034 May 01, 2012 - 22 - 7.0 ADDITIONAL SERVICES 7.1 Non-Scope Issues ASTM guidelines identify non-scope issues, which are beyond the scope of this practice. Non-scope issues have the potential to be business environmental risks. Some of these non-scope issues include; asbestos-containing building materials, radon, lead-based paint, lead in drinking water, wetlands and mold. We were authorized to conduct the following non-scope issues for the site: A soil gas vapor intrusion study was performed in conjunction with this study. Based on the documented plume of chlorinated solvents at the site and beneath the building, two soil gas vapor samples were collected along the north wall and one soil gas vapor sample was collected along the east wall. Three-eighth inch holes were bored in the concrete slab in these locations and soil gas samples were collected from beneath the floor slab using a new length of polyethylene tubing connected to a 6 liter Summa canister with a regulator set at 0.2 liters per minute. Laboratory analytical results have not been received at the time of this study. When the laboratory analytical results are received, the results of the soil gas sampling will be issued in a separate report. 7.2 Previous Reports Review We have not conducted previous environmental and/or engineering assessment activities at the site. We were provided with the following environmental and/or engineering assessment report(s) completed by others: We have reviewed several assessment reports provided by Mr. Joe Landry, the prospective developer of the site. Reports provided include A Monitoring Well Installation Report, prepared by Trigon Engineering Consultants, Inc. dated February 21, 1996; A Comprehensive Site Assessment Report prepared by Mineral Springs Environmental, P.C. (Mineral Springs), dated February 22, 2010; and a Comprehensive Site Assessment Addendum Report prepared by Mineral Springs Environmental, P.C. dated September 13, 2010. Our review of these reports is included in Section 5.2 of this report. ECS Project No. 09-21034 May 01, 2012 - 23 - 8.0 INTERVIEWS We spoke with Mr. Joe Landry with DM Squared, the potential purchaser of the site. According to Mr. Landry, the building at the site was constructed in the late 1960's and was occupied by a manufacturer of electronic devices. According to Mr. Landry, a release of chlorinated solvents has occurred at the site and environmental studies have been performed at the site. Mr. Landry has provided ECS with the results of these investigations. Our review of these reports are included in Section 5.2 of this report. Mr. Landry stated that based on the presence of a contaminant plume in the groundwater at the site, DM Squared is applying for admission into the NC Brownfields Program. According to Mr. Landry, DM Squared plans to renovate the building. DM Squared is a contract manufacturer of small to micro machines. Mr. Landry stated that the manufacturing process he employs uses solvents from 3M and do not contain chemicals that include PCE, TCE or any chlorinated compounds that are currently present in the groundwater at the site. Mr. Landry stated that the site receives water and sanitary sewer services provided by the City of King. Mr. Landry informed us that he was not aware of current or former petroleum containing USTs at the site. We spoke with Mr. Ron Hileman with King Investment Partners, the current owner of the site. According to Mr. Hileman, King Investment Partners was founded by a group of executives from Carolina Medical Electronics in 1984 and ownership of the property was transferred from Carolina Medical Electronics to King Investment Partners in 1985. According to Mr. Hileman, the site was owned by Carolina Medical Electronics from 1965 until 1985. Mr. Hileman stated that the building at the site was constructed in 1967 and has been remodeled and expanded periodically. Mr. Hileman stated that a release of chlorinated solvents has been documented at the site. According to Mr. Hileman, the release occurred from a discharge system and holding tanks installed to contain solvent wash. Mr. Hileman indicated that the contaminant plume has been delineated and at this time, there is no deed restriction or activity and land use restriction attached to the property deed. Mr. Hileman stated that the site has been connected to municipal water since at least 1985 and that water supply wells at the site have not been in use since at least 1985. Mr. Hileman indicated that in 2010, two water supply wells at properties to the south and southwest of the site were abandoned. Mr. Hileman informed us he was not aware of current or former petroleum containing USTs at the site. Neither Mr. Landry nor Mr. Hileman are aware of 1) environmental concerns associated with the site, 2) pending, past, or threatened administrative litigation or administrative proceedings relevant to hazardous substances or petroleum products in, on, or from the subject property or 3) government notices regarding any possible violation of environmental laws or possible liability related to hazardous substances or petroleum products. The NCDENR Winston-Salem Regional Office was unable to locate the incident file for this facility. ECS Project No. 09-21034 May 01, 2012 - 24 - 9.0 FINDINGS ECS was contracted by DM Squared to perform a Phase I ESA of a site located at 157 Industrial Drive, King, Stokes County, North Carolina. The subject site is an approximate 6.24 acre parcel. The site is located at 157 Industrial Drive in King, North Carolina. Based on a review of historical information, it appears the site was developed with a commercial building in 1967. The building at the site was periodically renovated and expanded. The site is developed with an approximate 13,440 square foot industrial building and asphalt paved areas. The building is currently unoccupied. The building contains office space, restrooms, manufacturing areas, storage areas, shipping and receiving and waiting areas. The building was formerly used to manufacture medical electronic devices. Light staining was observed on the concrete floor slab in several areas within the building. A floor drain and sump pump were observed in the northwestern portion of the building. The site currently receives water and sanitary sewer services from the City of King. The building is heated with electric heat pumps. Floor drains were observed beneath emergency wash stations and janitorial closets in the building. Several monitoring wells were observed north, east and west of the building. Water supply well caps were observed southeast of the building. The well house caps were locked. A former waste treatment system and drain field are located north of the building. We did not observe stressed vegetation, stained soils or evidence of underground storage tanks (USTs) at the site. The SER search provided by EDR identified Carolina Medical Electronics on the IMD. Laboratory analysis of groundwater samples collected from the monitoring wells detected several compounds in excess of the NC2L Standards. Tetrachloroethene (PCE) was the primary compound of concern. A PCE isoconcentration map indicates the contaminant plume extends across the majority of the site. The CSA is included in Appendix IV. According to a Comprehensive Site Assessment Addendum Report dated September 13, 2010, the NCDENR issued a correspondence requesting additional soil and groundwater assessment at the site. A meeting was conducted at the site in June 2010 and the NCDENR requested that the two water supply wells located south and west of the site be abandoned, installation of two additional monitoring wells south of the building, and collection of four soil samples in the vicinity of the former holding tank. The water supply wells were abandoned in June 2010. In July, 2010, two Type II monitoring wells (MW-10 and MW-11) were installed south of the building to further delineate the contaminant plume. Laboratory analysis of the groundwater samples collected from MW-10 and MW-11 did not detect the targeted compounds in excess of laboratory quantitation limits. Laboratory analysis of soil samples collected detected tetrachloroethene at a concentration below the NCDENR Industrial/Commercial Maximum Soil Contaminant Concentration (MSCC). The CSA and CSA addendum report recommend the completion of a Corrective Action Plan (CAP) to address groundwater impact at the site. The CAP has not been completed at the time of this study. The contaminant plume impacting groundwater at the site is considered to be a recognized environmental condition of the site. The SER search identified Parker Chevrolet, located approximately 2,150 feet southeast of the site as having a documented release of petroleum products. However, based on the distance and/or the groundwater flow direction inferred from the surface topography, the release from this property is not expected to impact the site. ECS Project No. 09-21034 May 01, 2012 - 26 - 10.0 CONCLUSIONS AND OPINIONS As documented and qualified by this report, this assessment has revealed the following recognized environmental conditions in connection with the property. On-Site A documented release of chlorinated solvents has been identified at the site. The contaminant plume has been vertically and horizontally delineated. The NCDENR has requested a Corrective Action Plan that has not been completed at this time. The contaminant plume impacting groundwater at the site is considered to be a recognized environmental condition of the site. It is likely that the incident at the site will be managed by the Inactive Hazardous SitesBranch (IHSB) section of the NCDENR in the future. The IHSB may require additional investigation. It is also possible that the solvent rinse could be considered hazardous waste. If so, the holding tanks could be considered Solid Waste Management Unit (SWMS) and could be regulated by RCRA depending on when the discharge began and ended. Additional considerations Based on the age of the building and the types of materials observed, asbestos containing building materials may be presentin the building. Prior to planned renovations and/or demolition of the building, ECS recommends an asbestos survey be performed on the building at the site. APPENDIX VI STATEMENT OF QUALIFICATIONS Mr. Stewart has conducted hydrogeologic site investigations at a variety of sites including NCDOT Right- of-Way corridors, Brownfields properties, landfills, industrial sites including textile and furniture plants, chemical manufacturing plants, commercial shopping facilities, dry cleaners, and gas stations. These studies have included the development of monitoring well networks, sampling plans, health and safety plans, writing comprehensive site assessments and corrective action plans. He has been directly involved in the implementation of pump and treat groundwater remediation projects, air sparging and vacuum extraction systems, treatment of contaminated soil and groundwater with biological and chemical additives, and monitored natural attenuation studies. Mr. Stewart has provided expert consulting in environmental litigation cases. He has managed multi-site NCDOT right-of-way and UST Section contracts. ENVIRONMENTAL SITE ASSESSMENTS - Mr. Stewart has managed numerous phase II environmental site assessments including both soil and groundwater investigations for Real Property Transfers. BROWNFIELDS - Mr. Stewart has managed private sector NC Brownfields sites and EPA funded Federal Brownfields sites. Types of Brownfields sites have included abandoned textile facilities, heavy machinery manufacturing and chemical manufacturing plants, and a former dump and incinerator.
