PURPOSE OF SITE VISIT: New Pumper Vehicle Re-Inspection
STATUS OF PAST NOTED VIOLATIONS:
1. North Carolina Administrative Code (NCAC) 13B .0844 (b) states: All permitted septage management firms shall display decals or lettering on each side of every pumper vehicle operated by the firm. The decals or lettering shall include the name, address (town name), phone number, and septage management firm permit number. All
decals or lettering on the pumper vehicle shall be no less than three inches in height and plainly visible. Identification shall be permanently attached (i.e., no removable signs).
On February 21, 2017. Cooke Rentals was in violation of NCAC 13B .0844 (b) because the address (town name) and the phone number was measured to be less than three inches in height on both sides of the pumper vehicle.
In order to come into compliance, Cooke Rentals was required to correct the lettering for the address (town name) and phone number on both sides of the pumper vehicle to measure at least 3 inches in height. After completion,
Cooke Rentals was required to submit a photograph of the completed lettering along with a ruler confirming the
corrections, to troy.harrison@ncdenr.gov by March 13, 2017.
On March 6, 2017, the photographs were submitted showing the required information. The violation is resolved.
2. The violation of North Carolina General Statute (NCGS) 130A-291.1 (h1) was resolved in the previous inspection
report.

ADDITIONAL COMMENTS
1. On February 21, 2017 an inspection of a new pumper vehicle (VIN IFDUF5GT5FEC84441) was conducted at
Cooke Rentals in Mt. Airy, Surry County, North Carolina. An observed violation was noted for improper lettering. On March 6, 2017, photographs of the corrected lettering was submitted to the Division of Waste Management/Solid Waste Section as requested. The lettering appears to have been corrected to be at least 3
inches in height.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
