The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The permitted facility consists of: One 225 gallon tank and three 5,300 gallon tanks for an approximate total
capacity of 16,125 gallons. The current permit modification was issued for the addition of a dissolved air flotation unit. Also observed were one, 20 yard dewatering box and one 30 yard dewatering box.
2. The facility is permitted for the storage and treatment of grease trap pumpings.
3. Wastewater from this facility is permitted to be discharged to the Archie Elledge WWTP. Solids from the
treatment process is permitted to be disposed at the Haynes Mill Road Landfill (Solid Waste Permit 34-02), Winston-Salem, Forsyth County, NC. Solid waste material is disposed at a permitted composting facility.
4. “No Trespassing” signs were posted on the exterior of the building at the time of inspection.
5. No objectionable odors were observed outside of the facility. 6. The permitted tanks appeared to be structurally sound and no leaks were observed. The dissolved air flotation
unit was also observed in operation.
7. It appeared that facility operations were being carried out as detailed in the approved Permitting and Operations
Manual for the facility.
8. No spills were observed on the ground outside of the building.
9. Facility logs were observed and appeared to be current.
10. The permit expires on April 24, 2017. If not already done, the company must immediately submit a permit renewal to the Solid Waste Section to maintain compliance. Please contact me if you have any questions or concerns regarding this inspection report.
