administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected new vehicle, 2600 gallon capacity, VIN#1FVACHDC46HV75659. Per Mr. Queen the tag to be
placed on vehicle will be #YA-55200.
2. The tank appears structurally sound and securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on both sides of this vehicle.
5. I contacted Mr. Queen by phone to inform him that the vehicle is in compliance.
6. Truck logs were not reviewed at this time.
7. At this time this is the only vehicle permitted for this firm. When you wish to bring your other vehicle into
service again please submit the additional fee ($250) to the Raleigh central office.
