Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected vehicle tag#YA136626, VIN# 1NKDL40X6BJ284162, 4000 gallon capacity. The tank appears structurally sound and securely attached. 2. The lettering is correct on both sides of this vehicle. 3. The outlet covers were on and no drips/leaks were noted. 4. I contacted the Raleigh Central Office that this vehicle is in compliance – in regards to the initial firm permit. No septage jobs shall be performed until notified that the firm permit has been issued. When septage jobs begin please start recording the required information for the truck pumping log – Ms. Willis stated that she had a log. 5. I gave Ms. Willis a 2017 class schedule – please attend one of the locations to obtain the required 4 hours of firm training for permit renewal this coming winter. 6. Discussed obtaining a septage detention facility permit – the required forms, setback distances, tank types etc. Enclosed is SDTF information.
